Step №2 - Documents for educational institution enrollment
Please provide a scan or photo of all the listed documents:
- Signed documents Oświadczenie Wzór 1, Wzór 2 (Download the documents from the LINK)
- Scan of the document confirming completion of basic secondary education
- Scan of the document confirming completion of full secondary education – if available;
- Scan of the certificate confirming knowledge of the Polish language at level B1 – if available
❗ After completing all steps, you will need to submit the originals of signed documents (Oświadczenie Wzór №1, №2) to your work Coordinator or send them to our office address:
