Fill out the online form and attach scans or photos of documents of the school completion and signed documents (Oświadczenie Wzór №1, №2).
❗ After completing all steps, you will need to submit the originals of signed documents (Oświadczenie Wzór №1, №2) to your work Coordinator or send them to our office address:
