'NHS Continuing Healthcare' means a package of continuing care arranged and funded solely by the NHS. The actual services provided as part of that package should be seen in the wider context of best practice and service development for each client group. The NHS Funded Care Team adhere to the National Framework for NHS Continuing Healthcare and the NHS-Funded Nursing Care June 2007, implemented October 2007. Continuing care funding deadlines On 15th March 2012, the Department of health announced the introduction of deadlines for new cases which require assessment of eligibility for NHS Continuing Healthcare funding.




A review is currently underway into ensuring that continuing healthcare is delivered fairly across the North West region. The NHS Funded Care Team The NHS Funded Care Team undertake and facilitate multi-disciplinary reviews of clients' needs, following comprehensive assessment of needs by professionals, carers and clients. The process is based on current documentation: o  Single Assessment Process (SAP) o  Care Programme Approach for Mental Health Patients o  Person Centred Plans for Learning Disability Clients All other NHS Trusts have responsibilities and duties in relation to continuing care and these duties include considering all clients over 18 for eligibility for NHS Continuing Healthcare (there are certain exceptions due to legislative responsibilities eg Section 117 of the Mental Health Act) The process for NHS Funded Continuing Care is not in itself an assessment, rather, it helps identify needs and particularly health needs. The main aim is to identify clients with a 'primary health need'.




This is identified through consideration of the 'Nature', the 'Intensity', the 'Complexity' and the 'Unpredictability' of the need. Any of these individually or in combination can indicate a 'primary health need'. NHS Cumbria is responsible for the identification of care needs following multi disciplinary team's assessments and decisions. NHS Cumbria is responsible for commissioning and contracting for all services for clients who qualify for NHS Continuing Healthcare. All clients in receipt of a package of care provided by Adult Social Care are means tested. The clients may also fund care privately and if their need is for healthcare then that may result in them paying for healthcare, which should be 'free at the point of need'. NHS Continuing Healthcare is not disease specific, nor determined by the setting of care or the provider of the care. NHS Funded Healthcare can be provided in: o  Patients' own homes Provision may be for: o  Single episode of care




o  Several episodes of care o  Remainder of life Eligibility is reassessed and reviewed on a regular basis according to patients' needs and government guidance. Based on clients' assessed health needs. Multi disciplinary team's make a recommendation based on the evidence available and forward their recommendation on a National Tool (the Decision Support Tool; which is a framework to assist decision making not a decision maker itself) to the Clinical Lead for the locality in which the clients' GP is based. This is then checked for evidence and completion, and then forwarded to the Cumbria NHS Funded Care panel for ratification.  Incomplete or unclear information may result in delays, or the overturning of the decision by the panel, this is only in exceptional circumstances. The process is not budget or finance led, it is very much a needs led process The team strive to ensure that the process is equitable As a Team we undertake all our training throughout all Cumbrian Health Trusts and Adult Social and Cultural Services. 




We also undertake presentations to the independent sector and voluntary organisations as requested, or where a need has been identified.
