Any condition that causes the glomerular walls to become inflamed is glomeru- lonephritis referred to as. One of the most common causes of glomerular inflammation is a reaction to the toxins given off by pathogenic bacteria, especially streptococci, that have recently infected another part of the body, usually the throat. Glomerulonephritis is also associated with autoim- mune diseases, such as systemic lupus erythematosus, polyarthritis, scleroderma, and diabetes.

When the glomerular membrane is inflamed, it becomes highly permeable. Red blood cells and protein pass through the glomerulus and enter the tubule as urine is being produced. In some cases, protein solidifies in the nephron tubules and forms solid masses that take the shape of the tubules in which they develop. These masses are called casts. They commonly pass out of the kidney by way of the urine and may be visible when urine is exam- ined microscopically. The clinical picture for this disease includes blood and protein in the urine (hematuria and proteinuria), and red cells casts, along with hypertension, edema, and impaired renal function. Most patients with acute glomerulonephritis associated with a strep- tococcal infection recover with no residual kidney damage





گلومرولونفریت (Glomerulonephritis)


هر شرایطی که باعث التهاب دیواره‌های گلومرولی شود، گلومرولونفریت نامیده می‌شود. یکی از شایع‌ترین علل التهاب گلومرولی، واکنش به سمومی است که توسط باکتری‌های بیماری‌زا، به‌ویژه استرپتوکوک‌ها، تولید می‌شود. این باکتری‌ها معمولاً پیش‌تر در بخش دیگری از بدن، مانند گلو، عفونت ایجاد کرده‌اند. گلومرولونفریت همچنین با بیماری‌های خودایمنی مانند لوپوس اریتماتوز سیستمیک، پلی‌آرتریت، اسکلرودرما و دیابت نیز مرتبط است.


زمانی که غشای گلومرولی دچار التهاب می‌شود، نفوذپذیری آن به شدت افزایش می‌یابد. در نتیجه، گلبول‌های قرمز خون و پروتئین از طریق گلومرول عبور کرده و وارد توبول‌ها می‌شوند، در حالی که ادرار در حال تولید است. در برخی موارد، پروتئین در لوله‌های نفرونی جامد شده و توده‌های جامدی را تشکیل می‌دهد که شکل لوله‌هایی را که در آن‌ها تشکیل شده‌اند، به خود می‌گیرند. این توده‌ها “کست” (Casts) نامیده می‌شوند. آن‌ها معمولاً از طریق ادرار از کلیه خارج شده و ممکن است در بررسی میکروسکوپی ادرار قابل مشاهده باشند.


تصویر بالینی این بیماری شامل وجود خون و پروتئین در ادرار (هماچوری و پروتئینوری)، وجود کست‌های گلبول قرمز، به‌همراه فشار خون بالا، ورم (ادم) و اختلال در عملکرد کلیه است. بیشتر بیماران مبتلا به گلومرولونفریت حاد که با عفونت استرپتوکوکی مرتبط است، بدون آسیب دائمی به کلیه، بهبودی کامل پیدا می‌کنند.
