Follicular lymphoma, a unique type of non-Hodgkin lymphoma that originates from B cells, a type of white blood cell, is known for its slow progression and the challenge it poses once it becomes aggressive.

What is Follicular Lymphoma?

Follicular lymphoma develops in the lymphatic system and affects the body’s ability to fight infections. It is characterised by the growth of abnormal B-cells in the lymph nodes, which can spread to other parts of the body, including the spleen, bone marrow, and other organs. Symptoms of follicular lymphoma include:

  • Painless swelling of the lymph nodes
  • Fatigue
  • Fever
  • Night sweats

For example, a patient diagnosed with follicular lymphoma may initially present with enlarged lymph nodes but might not experience significant discomfort until the disease progresses.

How Does Immunotherapy Work for Follicular Lymphoma?

Immunotherapy is a cutting-edge treatment that helps the immune system identify and attack cancer cells. Unlike chemotherapy, which indiscriminately kills rapidly dividing cells (including healthy cells), immunotherapy specifically targets cancer cells, minimising damage to healthy tissues.

  1. Monoclonal Antibodies: These laboratory-made molecules can bind to specific proteins on the surface of cancer cells, marking them for destruction by the immune system. Rituximab is one such monoclonal antibody used to treat follicular lymphoma.
  2. CAR T-Cell Therapy: This form of immunotherapy involves modifying a patient’s T-cells (a type of immune cell) to recognise and attack cancer cells. CAR T-cell therapy has shown promise in treating aggressive forms of follicular lymphoma that don’t respond to traditional treatments.
  3. Interferon Therapy: Interferon Alfa-2b, found in Intalfa 3MIU Injection, is a type of immunotherapy that boosts the immune system’s response to cancer. It works by enhancing the body’s natural defence mechanisms, allowing it to identify better and destroy cancer cells.

For example, a patient undergoing CAR T-cell therapy might have their T-cells extracted, modified in a laboratory, and then reinfused to more effectively target follicular lymphoma cells.

Advantages of Immunotherapy Over Traditional Treatments

  1. Targeted Action: Immunotherapy specifically targets cancer cells, leaving healthy cells largely unaffected. This reduces the overall toxicity of treatment and lowers the risk of side effects such as hair loss and nausea, which are common with chemotherapy.
  2. Long-Lasting Effects: Unlike chemotherapy, which often requires multiple cycles, immunotherapy can produce long-lasting results. Some immunotherapies, such as CAR T-cell therapy, continue to work even after the initial treatment, providing a more sustained defence against cancer.
  3. Personalised Treatment: Immunotherapy is often tailored to the individual patient. These are designed to enhance the patient’s immune system, making the treatment more personalised and potentially more effective.

Challenges and Side Effects of Immunotherapy for Follicular Lymphoma

While immunotherapy holds great promise, it has its challenges. Some patients may experience immune-related side effects, including:

  • Fatigue
  • Fever
  • Skin rashes
  • Inflammation of organs such as the lungs or liver

These side effects are typically managed with supportive care, but they can sometimes become severe, requiring adjustments to the treatment plan.

For example, a patient undergoing CAR T-cell therapy may develop cytokine release syndrome, a potentially life-threatening immune reaction that requires immediate medical attention.

As research in immunotherapy continues to evolve, new treatments are being developed that hold even greater potential. Clinical trials are ongoing to explore combinations of immunotherapies and other treatments, such as targeted therapies and radiotherapy.
