Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that facilitates research on pragmatic trials. It gathers and distributes clean trial data, ratings, and evaluations using PRECIS-2. This allows for a variety of meta-epidemiological studies to compare treatment effect estimates across trials of various levels of pragmatism.
Background
Pragmatic trials are becoming more widely recognized as providing real-world evidence for clinical decision making. However, the use of the term "pragmatic" is inconsistent and its definition as well as assessment requires clarification. Pragmatic trials are intended to guide clinical practices and policy choices, rather than confirm a physiological hypothesis or clinical hypothesis. A pragmatic study should try to be as similar to actual clinical practice as possible, including in its selection of participants, setting and design as well as the execution of the intervention, determination and analysis of outcomes and primary analysis. This is a major distinction from explanatory trials (as described by Schwartz and Lellouch1), which are intended to provide a more thorough proof of a hypothesis.
The most pragmatic trials should not blind participants or clinicians. This can result in an overestimation of treatment effects. The trials that are pragmatic should also try to recruit patients from a wide range of health care settings, so that their results can be compared to the real world.
Furthermore, pragmatic trials should focus on outcomes that are crucial to patients, such as quality of life or functional recovery. This is particularly important for trials involving surgical procedures that are invasive or have potentially serious adverse events. The CRASH trial29 compared a 2 page report with an electronic monitoring system for hospitalized patients with chronic cardiac failure. The catheter trial28, however, used symptomatic catheter associated urinary tract infection as the primary outcome.
In addition to these aspects, pragmatic trials should minimize the requirements for data collection and trial procedures to cut costs and time commitments. Finally, pragmatic trials should seek to make their findings as applicable to clinical practice as possible by ensuring that their primary analysis follows the intention-to treat approach (as described in CONSORT extensions for pragmatic trials).
Despite these guidelines however, a large number of RCTs with features that defy pragmatism have been incorrectly self-labeled pragmatic and published in journals of all types. This can result in misleading claims of pragmatism and the usage of the term should be standardized. The development of the PRECIS-2 tool, which offers an objective standard for assessing pragmatic features is a good initial step.
Methods
In a practical trial the goal is to inform clinical or policy decisions by showing how an intervention could be integrated into everyday routine care. This differs from explanation trials, which test hypotheses about the cause-effect connection in idealized settings. Therefore, pragmatic trials could have less internal validity than explanatory trials, and could be more susceptible to bias in their design, conduct and analysis. Despite these limitations, pragmatic trials can contribute valuable information to decision-making in healthcare.
The PRECIS-2 tool measures the degree of pragmatism within an RCT by assessing it across 9 domains that range from 1 (very explanatory) to 5 (very pragmatic). In this study, the recruit-ment, organization, flexibility in delivery and follow-up domains received high scores, but the primary outcome and the method of missing data fell below the practical limit. This suggests that a trial can be designed with effective practical features, yet not compromising its quality.
However, it's difficult to assess how practical a particular trial is, since pragmatism is not a binary characteristic; certain aspects of a trial can be more pragmatic than others. A trial's pragmatism can be affected by changes to the protocol or the logistics during the trial. Koppenaal and colleagues found that 36% of the 89 pragmatic studies were placebo-controlled, or conducted prior to the licensing. The majority of them were single-center. Thus, they are not as common and are only pragmatic when their sponsors are accepting of the lack of blinding in these trials.
Another common aspect of pragmatic trials is that the researchers attempt to make their findings more valuable by studying subgroups of the sample. This can lead to unbalanced comparisons with a lower statistical power, which increases the chance of not or misinterpreting the results of the primary outcome. In the instance of the pragmatic trials included in this meta-analysis this was a major issue because the secondary outcomes were not adjusted for the differences in baseline covariates.
In addition, pragmatic trials can also have challenges with respect to the gathering and interpretation of safety data. It is because adverse events are typically self-reported, and therefore are prone to delays, errors or coding errors. It is important to improve the quality and accuracy of the results in these trials.
Results
While the definition of pragmatism may not mean that trials must be 100% pragmatic, there are advantages to incorporating pragmatic components into clinical trials. These include:
Enhancing sensitivity to issues in the real world as well as reducing study size and cost, and enabling the trial results to be faster translated into actual clinical practice (by including patients who are routinely treated). However, pragmatic trials have disadvantages. The right type of heterogeneity, for example could allow a study to expand its findings to different settings or patients. However, the wrong type can reduce the sensitivity of an assay and thus decrease the ability of a study to detect minor treatment effects.
Several studies have attempted to classify pragmatic trials using a variety of definitions and scoring methods. Schwartz and Lellouch1 have developed a framework to distinguish between explanatory trials that confirm a physiological or clinical hypothesis as well as pragmatic trials that inform the selection of appropriate treatments in real-world clinical practice. The framework consisted of nine domains that were assessed on a scale of 1-5 which indicated that 1 was more informative and 5 was more pragmatic. The domains covered recruitment, setting up, delivery of intervention, flex adhering to the program and primary analysis.
The original PRECIS tool3 was based on a similar scale and domains. Koppenaal et al10 developed an adaptation of this assessment, dubbed the Pragmascope which was more user-friendly to use for systematic reviews. They discovered that pragmatic reviews scored higher on average in most domains, but scored lower in the primary analysis domain.
This difference in primary analysis domains could be due to the way in which most pragmatic trials analyze data. Some explanatory trials, however do not. The overall score was lower for pragmatic systematic reviews when the domains of organisation, flexible delivery, and follow-up were merged.
It is important to remember that a pragmatic trial does not necessarily mean a poor quality trial, and in fact there is an increasing number of clinical trials (as defined by MEDLINE search, but this is not specific or sensitive) that employ the term 'pragmatic' in their abstracts or titles. These terms could indicate a greater understanding of pragmatism in titles and abstracts, but it's unclear if this is reflected in content.
Conclusions
In recent times, pragmatic trials are increasing in popularity in research because the importance of real-world evidence is increasingly recognized. They are clinical trials randomized that compare real-world care alternatives instead of experimental treatments in development. They involve populations of patients that are more similar to the ones who are treated in routine care, they employ comparators which exist in routine practice (e.g. existing medications) and depend on participants' self-reports of outcomes. This method can help overcome the limitations of observational research like the biases that come with the reliance on volunteers, and the lack of the coding differences in national registry.
pragmatic kr have advantages, like the ability to draw on existing data sources, and a greater chance of detecting significant distinctions from traditional trials. However, they may still have limitations that undermine their reliability and generalizability. For example the participation rates in certain trials could be lower than anticipated due to the healthy-volunteer effect as well as financial incentives or competition for participants from other research studies (e.g., industry trials). Many pragmatic trials are also limited by the need to recruit participants quickly. Some pragmatic trials also lack controls to ensure that observed differences aren't caused by biases in the trial.
The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-labeled themselves as pragmatic and were published up to 2022. They assessed pragmatism by using the PRECIS-2 tool that includes the domains eligibility criteria, recruitment, flexibility in adherence to interventions and follow-up. They discovered that 14 of the trials scored highly or pragmatic practical (i.e., scoring 5 or more) in one or more of these domains, and that the majority were single-center.
Studies that have high pragmatism scores tend to have broader criteria for eligibility than traditional RCTs. They also include populations from many different hospitals. The authors claim that these characteristics can help make pragmatic trials more meaningful and relevant to everyday clinical practice, however they do not guarantee that a pragmatic trial is completely free of bias. Furthermore, the pragmatism of the trial is not a predetermined characteristic; a pragmatic trial that does not possess all the characteristics of a explanatory trial can yield reliable and relevant results.
