Before you're allowed to enter Lithuania, you will have to complete a Lithuania Registration Form. This is needed by all overseas residents of Lithuania, even though they might not yet have a resident visa. The Ministry of Foreign Affairs in Lithuania will allow you to obtain a visa so as to travel to Lithuania, however you'll still have to fill out the registration form before you can travel. You can still use your current passport as a valid form of identification

You also have to indicate whether or not you want a health insurance card. When it isn't a contagious disease that can spread easily, you will also need to explain if it has a particular name (i.e. strep throat). The only contagious diseases that require an entry to the vaccination centre's database are those that are automatically contracted through touch, inhalation, or blood.

List your marriage/divorce details. The only entry necessary for this part of your Lithuania Registration Type is the name. It's also a good idea to indicate if you were previously divorced or currently being married. The health authorities will also have to know the titles of any children or people that are residing with you. Be sure to record the legal names of these individuals too, including the town and city they reside in. Each these details should be in your registration form for Lithuania.
