Urinary Stones

قال رسول الله ﷺ : ( من قال : سبحان الله وبحمده في يوم مائة مرة حطت خطاياه وإن كان مثل زبد البحر ). 

بسم الله الحمدلله والصلاة والسلام على رسول الله. 

ال urinary stones

 حصوات الكلي منتشرة جدا نسبتها من 5 - 10 % 

يعني شعب مصر 100 مليون 

10 مليون منهم هيجيلهم urinary stones

وبتيجي ال male اكتر

🔴 The third most common affliction of the urinary tract 

تالت اشهر disease يصيب الكلى والجهاز البولي 

ييجي قبليها 

Exceeded only by: 

1- Urinary tract infection 

2- Pathologic conditions of the prostate 

دي كانت شوية معلومات وكدا 

ندخل بقا علي المهم

Etiology 

اي اللي ممكن يعملي stone ؟؟؟

• واحد مش بيشرب مية كتير فالمية في البول تقل فتزيد ال crystals وتتجمع مع بعض ويعملوا stone
• في انواع من ال food تزود ال stone لان بيبقى فيها مواد بتدخل في تكوين الحصوة دي يعني مثلاً 

Spanish - tomatoes - mango 

كل دول فيهم نسبة كبيرة من ال oxalate 

اللي لو زاد في البول يتجمعوا ويعملوا stone 

وبالمناسبة 

Ca oxalate commonest Ca stone.

• وعندك Liver - meats - salmon فيهم نسبة كبيرة من ال uric acid اللي برضو بيدخل في تكوين الحصوات

• في كمان endocrinal factors زي ال hyperparathyroidism بتزود الكالسيوم في الدم تعمل Hypercalcemia بالتالي hypercalciuria 

• و ال Renal tubular acidosis نفس النظام تزود ال bone resorption فتعلي الكالسيوم في الدم بالتالي في ال urine

• و ال Jejunoileal bypass دي عملية من ضمن عمليات التخسيس بيكون فيها

surgical anastomosis of the proximal portion of the jejunum connected to the distal portion of the ileum

ومن ضمن ال side effects بتاعتها انها بتـ enhance absorption of oxalate. 

بالتالي نسبة ca oxalate stones هتزيد 

• و ال Malabsorptive conditions مشاكل في الامتصاص هتؤدي إلي

🔴decrease urine volume

🔴decrease citrate absorption 

• ال citrate والماغنسيوم بيساهموا في تقليل تكوين الحصوات عشان كدا بنستخدمهم في ال medical therapy

بيقولك اي الحاجات اللي تزود نسبة حدوث الحصوات وايه اللي يقللها 

1🔴 low urine volume

2🔴 low ph 

إحنا عارفين ان البول slightly acidic اي تغيير في ال ph ممكن ينتج عنه stone 

Low ph. 

Uric acid stone تزود احتمالية 

high ph. 

تزود احتمالية struvite stone

Types of stones 

من حيث المكونات 😁

1🔴 Calcium stone  ( 85% of stones)

نتيجة زيادة الكالسيوم بكميات كبيرة في الجسم 

🍒hypercalciuria 3 types 

1_Absorbtive 

زيادة امتصاصها من الintestine

2_Resorbtive 

زيادة ال parathyroid hormone يزود ال bone resorption فيزود ال ca

hypercalciuria in hyperparathyroidism

 (5-10% of all stones) 

في الغالب بتكون بسبب adenoma in parathyroid وعلاجها  

removal of adenoma 

3_Renal induced renal tubular defect 

2🔴Calcium Oxalate Stones (75% of ca stones)

👀Ca oxalate stones is the commonest radiopaque stones 

حصوة spiky in shape بتجرح في ال mucosa وبتسبب bleeding عشان كدا لونها بيبقا brown to black

بتتكون في acidic urine

بتبان في ال x_ ray غامقة radiopaque

3🔴Non calcium stone

1-Struvite 

(Mg ammonium phosphate)

بتحصل بسبب infection in urinary tract

 urea splitting organism hydrolyze urea to ammonia thus increasing PH. 

زي

1_ Proteus 

- Pseudomonas 

- Klebsiella

Can make staghorn stones. 

2-Uric Acid Stones 

5% of urinary calculi

more in men 

Occur with gout

rapid Wt loss

cytotoxic drugs 

اي حاجه تسبب cell destruction 

او بياكل اكل فيه uric acid كتير 

(Radio-lucent)

عشان كدا مش بتبان في ال x ray 

يعني لو عملت اشعه لواحد عنده uric acid stone الاشعه هتطلع normal

وبالمناسبة 

👀 Uric acid stone is the commonest radiolucent stone

Treatment 

علاجها نخلي البول قاعدي عشان تتفتت 

ويشرب مية كتير عشان ال crystals لا تتجمع مع بعضها

✔️Alkalinization of urine  

✔️Plenty of fluids

4🔴Cystine stones  

- Uncommon 

- Inheritance is autosomal  

- Big stone volume and high recurrence 

بين قوسين " big khazook" 

- Genetic defect in tubular reabsorption leading to high amounts of cystine amino acids 

in urine.

Clinical presentation

العيان اللي عنده حصوات 

هيجيلك بايه؟

يجيلك بألم فظيع في جنبه

مش بيرتاح في اي وضعيه 

مريض الزايدة لو ميل شوية ممكن الالم يخف لكن مريض الحصوات مفيش حاجه بتريحه

Pain abrupt onset, severe, may awake from sleep, patient moves constant to get rid of pain

وال pain بتاع الحصوات نوعين 

• Renal colic

ودا بينتج من 

hyperperistalsis of ureter

And stretching of collecting system 

• Dull aching pain 

ودا بينتج من 

distension of renal capsule 

وال pain دا بيكون مصحوب معاه 

nausea and vomiting due to common innervation with git 

وكمان بيـ radiate to 

Testis in male 

And labia majora in female

حاحة تانية 

شدة الالم ومكانه بيعتمد على كام حاجة كدا 

1_ size of stone

2_ Location of stone

3_ degree of obstruction

4_ associated infection

5_ acuity of obstruction

كل م حجمها يكون أصغر كل م يكون الألم أشد

ممكن ميشتكيش من اي حاجه ونكتشف بالصدفة انه عنده stone

في المحاضرة مقسم الأعراض على حسب مكان ال stone 

🍒Renal calyx stone 

لو ال stone في ال calyx 

- Obstruction, colic, periodic, increase with excess fluids 

- deep dull aching 

- Mostly small 

🍒Renal Pelvis stone 

- Renal pain radiate to flank, confused With biliary colic (Rt side) 

🍒Upper, Mid ureter 

- Flank pain intermittent, mimic acute appendicitis 

🍒Distal ureter 

بيسمع في ال scrotum فبنلغبط بينه وبين ال testicular torsion

- Confused with torsion 

🍒Intramural Stone: 

في جدار الـ 

Bladder >> Cystitis

هييجي بـ 

 frequency , urgency , hematuria.

In female confused with menstrual pain and PID.

بيقولك تعمل اي لو واحدة حامل طلع عندها renal stone

_ هنتجنب الاشعاع والأدوية أول 3 شهور 

- لو هتعمل x ray لازم with sheild

 نمشيها conservative لغاية م تولد 

1_ نحطلها double j stent  

او 2 _ PCN

ندخل بقا على ال 

Investigation

اول حاجه تعملهاله ايه ؟ 

1🔴Urine analysis 

ممكن نلاقي 

RBCs 

Bacteria

Crystals

2🔴 US 

ممكن نشوف 

hydronephrosis

Stones

وحاجه تاني قالها الدكتور في الراوند 

مش بنشوف ال ureter في السونار إلا 

1_ proximal ureter

2_ intramural part in bladder

بنفضل نعمله في ال pregnant لانه آمن

ممكن كمان استفاد من السونار اني اشوف ال cortical thinning اعرف من خلاله الكلية شغالة ولا لا

Normal cortical thinning= 20_ 25 cm

3🔴 X Ray

_Can detect only Radio-opaque stones 

_Can be used for follow up during treatment of radio-opaque ureteric stones 

_ Can detect calcifications of the urinary tract

4🔴 IVU 

intravenous urography

_Injection of a dye through the vein to visualize the pelvi calyceal system 

بنشوف منه ال 

_Anatomy of kidney 

 _function of urinary tract (excretion of the kidney)

لو الكلية بايظة معني كدا انها مش هتقد تعمل excretion للصبغة دي 

_ congenital anomalies

_ stone location 

في حالة ال renal colic بيحصل spasm of vessel بالتالي الصبغة مش هتوصل لل kidney فممكن يلغبطني وافتكر انه الكلية مش شغالة وأبدأ اتصرف على اساس كدا 

لكن من البداية كان لازم تديله مسكن عشان ميحصلش spasm والصبغة تعرف تعدي 

_Radiolucent stones can be visualized as filling defects in IVU 

🍒Contraindications 

• Contrast allergy 
• With high Serum creatinine . 

لان دا يدل ان ال kidney مش شغالة

• Metformin hydrochloride (oral antihyperglycemic) Glucophage

بياخد دوا للسكر يعني عنده DM يعني احتمال يكون دخل في complications وفي الاخر يطلع عنده nephropathy 

• Multiple myelomatosis

5🔴 CT 

🌿 CT KUB non contrast with acute renal colic: 

في حالة ال sever renal colic ومينفعش اديله صبغة اعمل ال ct without contrast

_No IV contrast, very accurate in detecting stones 

_Can show all stones including radio-lucent stone.

_ Assess stone density by the Hounsfield Unit (HU) 

_ Can detect skin to stone distance

🌿 CT with contrast can be used in complex renal anatomy: 

🌿 Retrograde pyelography: 

🌿 Antegrade pyelography (Nephrostogram): 

🌿 Magnetic Resonance Urography (MRU): 

_Can detect level of obstruction not the stone  

_Has no radiation hazard with pregnancy 

🌿 Radio- isotope study 

 In poor functioning kidney to asess kidney function prior to treatment 

 For medico-legal purpose

البدف ملغبط خالص بصراحة ويقرف

علشان نعالج حصوات الكلى قدامنا كذا اختيار 

1_ conservative

2_ ESWL

3_PCNL

4_ Ureteroscopy

5_ Surgical 

ناخد كل واحد ونتكلم عنه بالتفصيل ونعرف امتى نختار كل واحد فيهم 

1- Conservative

ازاي اقدر اعالج حصوات الكلى medical

✔️ Fluid intake

✔️ Analgesia

_ first choice is NSAIDs 

_ second choice is opioid 

✔️alkalinization of urine 

في حالة ال uric acid stone

Alkalnizing agent 

1. Na bicarb 

2. K citrate

✔️ acidification of urine

في حالة ال struvite stone اللي بتنتج بسبب اللي ال infection اللي بتحول ال urea into ammonia 

_ alpha blocker >> smooth muscle relaxant

بستخدمها في حالة ال lower ureter stone

_ penicillamine and alpha mercapto propronyl glycin 

في حالة cystine stone

سؤال امتى اعالج conservative

1. If stone size < 5mm

2.No back pressure

3. Not associated with infection

خلصنا ال conservative اللهم لك الحمد

2- ESWL 

اختصار لـ Extra corporeal shock wave lithotripsy 

عبارة عن جهاز ع شكل حرف c بيطلع ultrasound shock wave بيكسر بيها ال stone 

طيب امتى استخدمه 

1. Stone too large to pass through kidney

2. Size of stone in kidney< 2cm 

3. Size of stone in ureter < 1cm

4. No distal obstruction

No bleeding tendency 

لان من ال side effect بتاعته انه بيعمل hematoma

3- PCNL

اختصار لـ percutaneous nephro lithotripsy 

بدخل بمنظار من خلال فتحة في ال flank 

Under sonar guide 

لغاية م اوصل الي ال renal stone

وبعد كدا افتت الحصوة دي وازيلها دا كله بالمنظار lithotripsy

امتى استخدمه ؟ 

1. can't use ESWL

2. if stone> 2 cm

3. in case of renal and upper ureteric stone

4- Ureteroscopy

منظار الحالب منه نوعين

Rigid

في حالة distal ureter stone 

Flexible

اسمه RIRS 

اختصار لـ retrograde intra renal surgery

يعني بندخل المنظار من تحت لفوق من ال urethra to ureter

في حالة proximal ureter stone

And renal stone

5- Open surgery

في حالة لو كل اللي فوق دول فشلوا 

معلومة قالها الدكتور في الراوند 

امتى نعمل surgery 

لو كان 

2 hard stone

2 or multiple stone 

High density stone

_ pyelolithotomy

لو ال stone موجودة في ال renal pelvis

_Nephrolithotomy 

لو موجودة في ال renal parenchyma

اسم الفتحة اللي بفتحها في ال skin عشان اشيل الحصوة morithon incision

بس كدا الحمدلله رب العالمين

Bladder stone 

Urethral stone
