All foreign tourists who want to travel to Tunisia must complete the Tunisia Health Declaration Form. This document is required to help at least prevent the spread of Middle East respiratory Syndrome (or MRS), a deadly disease that has swept through Europe and the Middle East. This form should be electronically signed, and printed. You can also hand it to a Tunisian health authority upon your arrival. This is essentially a two page form that requests basic information regarding your health history.
