Some prosthetics (“stand to pee” or STPs) can be used to aid a man with a vulva in peeing at urinals standing up. Many STP prosthetics also achieve adequate cosmetic results. Some prosthetics (“pack and play”) are designed to both serve as an everyday cosmetic packer and to be capable of becoming stiff enough to use for fellatio or penetrative sex. Some prosthetics (“strap ons”) are used primarily or purely for sexual purposes, serving the function of a fully erect penis.
With testosterone treatment, there is typically at least some “clitoral hypertrophy”, meaning that the erectile tissue near the urethra and throughout the rest of the genital area becomes larger, more visually prominent, and it's erections become unmistakably visible. The degree to which this occurs varies greatly from one man to another, depending on their individual bodies' responses, and possibly to some as yet unknown inheritable factor. It is certainly possible to experience enough “bottom growth” that he has a penis. Often quite small by the standards of typical cis male anatomy, but visually and functionally a penis. It becomes erect on its own, and if big enough can be used in penetrative sex. One significant difference between cis men's penises and the larger end of trans men's penises is that in the latter case, the urethral opening is at the base, not the tip. Minimally invasive surgeries can lengthen the urethra to correct that difference. Trans men's penises of this sort have folds that function as and look like foreskins.

In many places, a Transgender person does not require full bottom surgery in order to be legally recognised as their true gender and many do not opt for this invasive and painful surgery.
Also, with the use of Testosterone, many Trans guys find that their clitoris grows and extends in length to give them the appearance of a small penis and often this is enough for some guys. However, some trans men feel that they would like to opt for the full surgery in order to feel ‘complete’ as a man. This is entirely personal preference and it is down to the individual to make that decision for themselves and all Trans men are men, regardless of whether or not they have had the bottom surgery.
It is also important to stress that you NEVER ask a Transgender person if they have had surgery or not as that is absolutely non of your business, unless they wish to share that with you.
There are a number of options available for a Trans guy wishing to have bottom Surgery.
There are many risks involved with the surgery however for many Trans guys, it is something that they want so much more and it outweighs the risks involved.
