Many serious accidents have occurred due to machines started to run when it was supposed to be completely off for maintenance or repairs. These incidents are usually caused by stored energy that is not properly released. Lock-out tag-out (LOTO) systems can prevent these incidents by ensuring that machines remain completely off until the person who is authorized releases them.

The LOTO system is comprised of a series of locks tags, locks, and other equipment that are used to isolate, secure or shut off the machine from an electrical source. All "affected", "authorized" and "qualified" personnel should be trained on the use of these devices. These devices include, but are not restricted to chains, locks, and padlocks. Each device is identified and labeled, indicating the date and the time it was applied, the name and title of the person who was authorized to use it and the reason for the lockout/tagout (repairs maintenance, repairs or repairs, etc.). It must also be clearly stated that these devices cannot be removed until a thorough LOTO procedure has been completed.
