I second the recommendations for lumpinou's lgbtqa mod, I believe it lets you send sims for top surgey. Zero has a mental health mod which includes gender dysphoria and options for sims to seek support for gender dysphoria, as well as an HRT mod which is intended to work alongside the mental health mod.
