Another porn star who worked in the US adult entertainment industry has been found dead.
Olivia Lua, 23, died in a rehab facility in West Hollywood on Friday morning.
The actress, who used the screen name Olivia Voltaire, is believed to have died from mixing prescription drugs and alcohol .
It is not known how she got the substances into the facility where she was being treated for addiction.
Her agency, LA Direct Models, confirmed her death to XBiz , the porn industry's news outlet, on Friday.
Olivia's death is the fifth in just six months across the US and Canada. Four other women have died from suicide, accidental overdose and UTIs.

In his statement, Derek Hay of L.A. Direct Models said Lua had been battling addiction for some time and was seeking help.
He said it was a 'coincidence' that the two most recent deaths were of women who belonged to L.A. Direct Models.
'Much comment has recently been made on the number of adult stars having passed in the last year and with great sadness we must inform that the list has grown longer.
'Olivia joined LA Direct Models in April 2017, though had not been available for work since early October of last year, as she had been facing some personal challenges that had seen her in residential rehab for a period of nearly three months and from which she had been out for about a month, hoping to make a return to work in the early part of this year.
'We learned today that she had returned to a different facility in West Hollywood after a relapse, approximately a week ago, at which she was found deceased this morning.
Lua had been seeking help for substance abuse problems. Before she died, she tweeted a final selfie (left) and said: 'No one alive can be an angel forever'
Olivia's agency said they were shocked by her death. She had not worked for some time due to her addiction problems
'Family and close friends [said] they had deep concern at the volume of prescription drugs prescribed to Olivia and the danger this posed [to] her. If this was mixed with recreational drugs or alcohol, it is believed this to be the cause of her passing,' he said.
Olivia Nova, who died in Las Vegas earlier this month aged 20 from sepsis that is believed to have been caused by a UTI, also worked for LA Direct Models.
Hay said it was merely a 'coincidence' that the two most recent deaths were of women who belonged to his agency.
'We at Direct Models obviously can barely believe we are issuing a notice such as this, not once but twice, in such a short space of time.
'The coincidence that both of these models chosen stage names is Olivia is solely that and they otherwise have no connection. We actually do not think they had ever met.'

Days before her death, Lua shared cryptic social media posts including one which read 'no one alive can be an angel forever'.
It is not clear if any of the five models who have died recently knew one another.
So far, there has been no proof of a link between any of their deaths beyond the fact that they were all porn stars.
The first death was that of 35-year-old Shyla Stylez. Stylez died in her sleep at her mother's home in Calgary on November 9. So far, no reason has been given for her death.
Stylez left the porn industry in 2016 after 10 years.
In December, actress August Ames, 23, hanged herself in California, where she lived with her husband.
What prompted her suicide remains unclear but in the days beforehand, she found herself in the center of a firestorm on Twitter after publicly saying that she had refused to work with a man she knew did both straight and gay scenes.

Ames' argument was that because the male actor worked with both men and women, he was more likely to have contracted STIs and she believed it put her at risk.
Days after Ames' death, 31-year-old Yurizan Beltran died of an apparent drug overdose at home in California. It is not known if the overdose was accidental or intentional.
On January 9, 20-year-old Olivia Nova died in her bed in Las Vegas.
The young actress had been tweeting about how she was alone for the holidays and wanted to call a fan to lift her mood.
At first, it was suspected that drugs or alcohol may have played a part in her death.
However police reports which have emerged recently suggest the woman was suffering from sepsis.
She had prescription medication given for a urinary tract infection at the home when she died, according to the report.

We’ve all heard that some people just seem to be “born leaders” in life. While I’d argue that the vast majority of us serve a leadership role in at least a few aspects in our own existence, it would tempt the ludicrous to maintain that we each have similar abilities in this area. Some people we’re fortunate enough to run across in our own little circles of being can absolutely dominate a room, just by walking into it.
I won’t belabor obvious points about sex appeal, appearance, and charm here, because obviously if you happen to be reading this, you’ve already acquainted yourself with Shayla’s allure. We do find ourselves sitting inside her Members’ Area, after all. Most of you probably don’t realize, though, that during the decade plus that a Risqué Business sat atop the adult industry talent management pool, Shayla served as the unofficial Leader and Queen of the enterprise. She didn’t have any fancy title that went along with the duties, and honestly we never really talked about it much, but every one of us knew that we’d be much better off if we ran any major decisions through Shayla. In historic terms, for those of you not too young to appreciate history yet, we all had Shayla as our Matriarch.
Since Shayla has wandered back into the fold (as it were), at least as much as Matriarchs “wander” anywhere, we now have a chance to not only talk about her evolution from Strip Tease company owner, to Hall of Fame Porn Star, to … well, the term “mom” came to mind, but I didn’t want to get in trouble here. We all have our own opinions regarding where Shayla ended up, so you can fill in your own blank there. Besides, Shayla has been maternal with all the other girls in the group from the time she stared in this business in her early 20’s. (She was almost certainly maternal before that too, but I didn’t know her, so I can’t say. Drat!) Without delving any further into the whole Oedipal thing, I should probably just move along.
Suffice it to say that without Shayla, the Risqué World would have been a markedly different, and inferior, place. So even when we decided to do our third “Essentially” movie for Ashton View, we knew that we needed Shayla to help the process along. We hope to have time to relate a lot of the “theory behind the practice” sorts of moments on shayla.com here, clearly, but for now the folks here decided that we should start with Essentially Dee and the 4-girl orgy.

Over the years we’ve received a ton of emails about how people would rather have more frequent, shorter, posts by the way. Eventually we even learned to listen to our Members. Go figure. So we’ve divided the scene between the four lovely ladies (coincidentally all Risqué clients – imagine) into two parts. Apparently people have other things to do in their lives besides spending all their time looking the risqué commune sites, although I can’t imagine what that might be.
Now when you watch this first part, try to pay attention to Shayla’s very quiet “theoretically background role” in the event. Juli and I owned the company. The movie had Dee’s name in the title. Yet we all sort of naturally revolved around Shayla. No one else in the office will have read this article before it goes live, and since I haven’t shared these opinions with anyone there, I’ll be interested to see if anyone else can pick up on the nuance. Most of us tend to watch movies or binge tv shows before we sleep, and naturally people ask us about our current intrigues. Now we have a relatively diverse group of people at Risqué so the collection may seem a bit bizarre. Let’s be honest, though. This will hardly be the strangest thing you learn about us if you join .

Porn star Dahlia Sky was found dead in her car from an apparent gunshot wound on Wednesday. She was 31.
The 10-year veteran of the adult entertainment industry would have observed her 32nd birthday on Aug. 10, according to Adult Video News .
Police are looking into the case as a potential suicide. The adult industry performer had allegedly been diagnosed with terminal breast cancer and was battling depression at the time of her death, the Sun reported .
Sky’s body was found in the vehicle at approximately 8 p.m. June 30 in the San Fernando Valley’s Devonshire area, LAPD detective Dave Peteque told AVN. Police are awaiting autopsy results from the Los Angeles County Department of Medical Examiner-Coroner.
While no formal cause of death has been recorded, Peteque said “potential suicide” is “what the investigation is leaning towards, and there’s no evidence at this time that there’s any foul play.”
Since her passing, friends and fans have come forward to memorialize Sky’s prolific career and nuanced personality.
“During her last year I spoke to her several times about life. It was not an easy ride for her,” said Hans — the mononymous owner of JHP Films, which co-produced some of Sky’s previous work — in a statement to AVN. “This will take a long time to get over, if it’s even possible. Dahlia Sky, you beautiful soul, you funny, complicated and warm-hearted friend! I am going to miss you so much.”
Sky began adult entertainment work in 2010 under the name Bailey Blue — she changed it in 2014 following a cease-and-desist from a clothing company of the same name, according to AVN — and proceeded to accrue more than 600 credits, as well as various industry awards. She was open about her battle with stage 4 metastatic breast cancer and depression on social media.
“She was super funny and she had a big heart, but unfortunately her battle with breast cancer took a toll on her mental health and she struggled with depression, as most people would,” producer Axel Braun told AVN of Sky. “My heart goes out to her family and loved ones, we lost a beautiful soul.”
