In the context of the episode (as well as the show), a puffy vag*na does not mean anything significant. It was just something that Rick said in an impromptu way to make a puffy vag*na sound unappealing and something horrible to Morty (when it is actually innocuous and even awesome for some!). Morty was clearly surprised as he said, “What’s wrong with that? That doesn’t sound like a problem to me.” A puffy vag*na sounded fine to Morty because he did not have any experience with vag*nas to have any inclinations, which was exactly what Rick played with. Rick said, “puffy vag*na” as if it was a detriment, a justifying reason to not let Annie stick around. It’s more a character personality joke than a dialogue joke – a joke about Morty’s inexperience.
The “puffy vag*na” joke was not related to Hepatitis C or Rick being a pervert. Behind it is an amusing story about how the “puffy vag*na” joke was included in Rick and Morty Season 1 Episode 3. One of the show’s creators, Dan Harmon, inserted the “puffy vag*na” line into the script as a placeholder to make the other writers laugh. It was something he did in a light spirit and expected the other writers to change it as the network would not like it. He overlooked the fact that the network was Adult Swim, where jokes of such nature were pretty much allowed, and that the showrunner was Justin Roiland, who was fine with this type of humor and was known for even more.
