4 Plastic Surgeries Guys Are Getting to Look More Manly

You know the body you’re born with is not the body you’re stuck with—as long as you’re willing to put in the time and effort . But your face? Well, you’re kind of stuck with that.
As a guy, you might think your face-fixing options are limited compared to ladies’ options: It’s far more common for American women to utilize makeup, skincare, Botox, plastic surgery , and at least 100 beauty treatments of which you’ve never heard. Meanwhile, you’ve got the ability to grow facial hair . Maybe.
But here’s the thing: Men actually have all the same options that women do (there’s even makeup for guys )—you just don’t use them. And while you may not be exploring your options, lots of other men are. In 2015, more than 1.2 million cosmetic procedures were performed on male patients—an increase of more than 325% since 1997, according to the American Society for Aesthetic Plastic Surgery . Women still dominate the customer base, but men make up almost 10% of patients for both nonsurgical procedures (like dermal fillers, shown above) and surgical procedures.
Of course, plastic surgery is very different for men than it is for women, especially around the face. So we sat down with Douglas Steinbrech, M.D., a board-certified plastic surgeon who specializes in male plastic surgery, to discuss what guys are looking for when they go under the knife.

There’s not much you can do to change your jawline naturally if you’re already at a healthy weight— these jaw exercises can help a little, but they’re not going to turn you into Colton Haynes . The good news is that a weaker jawline isn’t necessarily a bad thing. Studies show that women don’t necessarily want a guy with a ridiculously masculine face.
Still, a lot of men are looking for a stronger, more sharply defined jawline, especially if they work out. “I see a lot of personal trainers who want a more ‘athletic’ contour to the jawline,” Steinbrech says. “People expect personal trainers to look ripped, and a strong jaw makes you look fitter.”
There are two ways to get a stronger jawline: dermal fillers (as shown in this photo) and jaw implants . Dermal fillers are gels made of hyaluronic acid (a natural compound in your body), and are injected into your face. Fillers are cheaper than surgery, and the procedure is easy to perform—Steinbrech says he can do an entire “model makeover” with fillers in about 15 minutes—but they’re also temporary. Fillers last around six to nine months, depending upon how quickly your body breaks down the acid. Implants, on the other hand, are permanent, but the process is more involved: The doctor makes a small incision in your cheek (inside your mouth, so no scars are visible) and places a small implant in your jaw.
Jaw implants are sometimes coupled with a chin implant to square out the whole jawline. And don’t worry about losing the cleft in your chin, because surgery is a step ahead of you. “If you’ve got a dimple or a cleft in your chin, or you want one, we have custom implants for that,” Steinbrech says.
Ultra-masculine cartoon characters—think Joe from Family Guy —are often drawn with exaggerated jaws twice the width of their faces. But this isn’t a good look on real people. A masculine face is squared off and proportional, which means your jaw and your forehead should be equally broad. This might not be a big issue for many guys, but Steinbrech says it starts to become a more common problem in men with extremely low body fat. “You do have some fat in your temples and in your forehead,” Steinbrech explains. “When you get really ripped, the loss of fat starts to show, and you start to look sickly.”
As with your jaw, there are two options for fixing this: dermal fillers (as shown here) and implants . If you go with fillers, Steinbrech says, it’s important to look for a doctor who has a lot of male patients—or you might end up looking overdone. “A lot of plastic surgeons do the same thing for men that they would do for women,” Steinbrech says. “But men should have some wrinkles, lines—you want it to be fuller, but not more feminine.”
Elective rhinoplasty, aka a nose job, is one of the most popular surgical procedures for men, according to a 2016 report from the American Society for Aesthetic Plastic Surgery . There’s no exact answer as to what makes a nose masculine and attractive, because an attractive nose is proportional to the rest of your face—what works for one guy might not work for another.
Generally, Steinbrech says a masculine nose is straight and refined, but not too small. Finding the ideal surgeon is always important, but it’s especially important for a nose job. “You only want to get one nose job,” Steinbrech says. “Corrective surgery is always harder than doing it right the first time, and the more ‘overdone’ a nose is, the more feminine it starts to look.”
“Baby face” isn’t usually synonymous with “manly”. For the most part, your cheeks are genetic—some people just have a little extra fat there, and it won’t go away naturally, not even if you’re at single-digit body fat.
“This is another surgery a lot of bodybuilders are looking for,” Steinbrech says. “It just looks comical when your face doesn’t match your body, like all those ‘don’t skip head day’ memes.”
To fix this, guys can get cheek implants or buccal fat removal (or both). Cheek implants change your bone structure—they can give you higher, more prominent cheekbones, and make your cheeks look less round. To figure out where you cheekbones are, place your thumb just above your tragus (the little nub inside your ear) and place your forefinger below your nostril. Draw your thumb and forefinger together until they meet in the middle. If the center of that line is closer to your eye, you have high cheekbones; if it’s closer to the bottom of your nose, you have low cheekbones. If you’ve already got high cheekbones and you still look like you’re 12, a surgeon can reduce your “buccal” (lower cheek) fat. Buccal fat removal is a simple procedure, but it’s not always recommended, especially if you’re younger, because sometimes aging alone will reduce the fat there.
