Considering that the vagina is designed to stretch so much that another human can pass through it, you'd think it could easily fit any size erect penis. At rest, the vaginal canal measures 3.7 inches long, on average. But when you're sexually aroused, it elongates, lubricates, and expands—so even sex with an XL guy is slippery and comfy.
But as it turns out, sometimes a penis can be too big to fit inside a vagina.
“It’s a very rare situation, [but] there are some men that are so endowed it makes certain positions difficult or painful,” Christine Greves, ob-gyn at the center for obstetrics and gynecology at Orlando Health, tells Health . The average erect penis comes in at 5.1 inches, but even a man who is a couple of inches larger can be hard to, well, accommodate. (For the record, the largest erect penis clocks in at more than 13 inches...yikes.)
Another penis-vagina fit issue has to do with a woman's age. In your 20s, 30s, and 40s, your estrogen levels are naturally high, which keeps vaginal tissue supple. That makes the vagina pretty expandable and less sensitive, says Dr. Greves.
But once you hit menopause, your body's estrogen output takes a dive. With less natural lubrication, an average or larger penis might have a tough time time fitting in without causing pain and friction for both the man and the woman.
It's also possible that just as some penises are on the small side, some vaginas might be, too. An exceptionally large penis, then, would be a mismatch for a small vagina. “Some vaginas are larger than others...so I would think it’s based on an individual basis," said Dr. Greves.
Okay, what are you supposed to if your v isn't comfortably accommodating his p? Extra foreplay can help; the more aroused you are, the more lubrication your body can produce, which can make things slippery and stretchy. Store-bought lube is also a good idea.
Stick to sex positions that put you in control, like woman on top or reverse cowgirl. When you're in charge of the depth and speed, he won't be able to go so deep or thrust so fast that it doesn't feel good. Positions that keep your legs wide can also help, since that can elongate the vagina.
If the problem stems from menopause, “what I recommend for my post-estrogen women is an estrogen cream and a vaginal dilator. Try to get a dilator similar to the size of the man. And with lubrication, you could see if that could accommodate [the penis],” Dr. Greves says. If you've tried several tactics but nothing seems to work, it can't hurt to check in with your ob-gyn, she advises.

Priscilla Fleming became a licensed massage therapist in 2019 to help people. What she didn’t expect was sexual harassment, which she says began almost instantly. “At that point I now had to process this traumatic experience while also navigating a brand new industry that put me alone in a dark room with strangers. She’s also on a mission to combat harmful stereotypes that plague the massage industry. The dangers facing massage therapists made headlines last month, when NFL quarterback Deshuan Watson was suspended by the Cleveland Browns for 11 games and given a $5 million fine after he was accused of sexual misconduct by 24 massage therapists. Allegations included Watson exposing himself and manipulating therapists into touching him in an inappropriate manner. Two of the women also accused Watson of pressuring them to perform oral sex. While Watson has repeatedly denied the claims, 23 of the 24 civil lawsuits have been settled. In an interview with Sports Radio 610, Watson’s lawyer, Rusty Hardin, claimed that a ‘happy ending’ was not a crime unless extra money was paid for the service. “I feel as though he single handedly put a lot of us at risk to be assaulted,” says Fleming. “The NFL is a very large, well known industry and there is a very large fan base. So by his lawyer making these allegations, I'm afraid that this is going to empower that fan base to come and seek what Dashaun Watson was receiving.” With her ethics course, Fleming spends considerable time educating other therapists about gaslighting and grooming tactics used by predatory clients. Gaslighting involves manipulating someone by sowing self doubt in what they are experiencing, and grooming is a process of seeing how far a predator can push past a person's personal boundaries. Fleming notes that in the therapeutic relationship, the licensed therapist is granted the power to lead the dynamic in a professional setting. She says that when that power dynamic shifts, therapists may find themselves operating in threatening territory.

The name says it all. "Penis captivus" is the medical term for when the penis is essentially held captive by the vagina during sexual intercourse. It gets stuck, neither partner can separate the bond, and this is not a myth. But for as terrifying as it sounds, does its severity really live up to the hype?
The idea of a penis getting stuck in a vagina might bring to mind the 2007 film Teeth , a comically absurd horror film about a teen girl whose vagina houses teeth — described in folk tales as vagina dentata — which sever every unknowing man’s member, especially when he’s committed a wrong (in her mind). Thankfully, penis captivus isn’t as gruesome, as common, or as serious.
“When the penis is within the vagina, it becomes increasingly engorged,” Dr. John Dean, clinical director of gender and sexual medicine for the Devon Partnership NHS Trust in southwest England, recently told BBC Health Check . “The muscles of the woman’s pelvic floor contract rhythmically at orgasm… While those muscles contract, the penis becomes stuck and further engorged within [the vagina] until the muscles relax. Blood can flow out of the penis again, the penis starts to go down after orgasm, and the man can withdraw.” He also noted that the rhythmic contractions a woman experiences during an orgasm aren’t spasms — these usually characterize vaginismus , which can cause vaginal spasms so strong and painful that the vagina closes, according to the National Institutes of Health .
The fascination with penis captivus, Dean said, is the “prospect of a couple struggling to separate themselves for many minutes.” But he says that in his own experience, on the rare occasions that he’s heard of it, it only happens for a few seconds — between five and 10. Long enough that “if you’re in that situation that probably seems like an eternity.”
Although Dean passes penis captivus off as a mild occurrence, there have been reports of more severe cases, however, the condition is already so rare. Only a handful have been reported in the past century, and many were dismissed because they were “based on hearsay” with a “tenuous connection” to the “actual facts,” Dr. F. Kraupl Taylor, a physician, wrote in a 1979 article in the British Medical Journal (BMJ) . But of the cases that he speaks about, he mentions a “more or less credible instance” from 1910, in which a pair found themselves stuck, and in “intense pain,” from attempts to separate. It wasn’t until the woman was given chloroform, which was once used as an anesthetic, that her vagina was able to release the man’s penis.
Another physician’s response to Taylor was published in BMJ in 1980. Dr. Brendan Musgrave’s letter spoke about a personal experience that occurred in 1947. At the time, Musgrave was a houseman at the Royal Isle of Wight County Hospital in south England, when a honeymoon couple was brought out of an ambulance “on a single stretcher.” The woman was also given anesthetic, and the couple were discharged.
Penis captivus occurs more frequently in animals than it does in humans, Dean said. And it’s not uncommon to see animals that “are stuck together after sex, particularly dogs … and people look on with bemusement.” If there was to be a more notable, severe case in humans that necessitated intervention or admission to a hospital, Taylor wrote, “it would have been eagerly reported in a medical journal with as much evidence and detail as possible.”
