Cock extenders have helped many men when it comes to erectile dysfunction and size.
All too often, our penises don’t work the way we want them to. We can’t get or maintain an erection , even though we’re really aroused. Then, of course, many cisgender guys wish they had a longer and thicker penis .
If you’ve ever struggled with erectile dysfunction (ED) or you’re packing on the much smaller side , you may have heard of a penis sleeve, or perhaps one of the many other names for the revolutionary device: cock sleeve, penis extender, external penile prosthetic, or penis sheath. It’s likely left you thinking: what exactly is this bizarre contraption, how does it work, and do I need (or want) one?
Well, to start, “Penis sheaths are devices that go over the penis, in order to modify its ability,” says Michael Ingber , M.D., a urologist and urogynecologist at Garden State Urology. “There are varying types. Those which add length in men who feel a bit too short, those which add girth in those who want a thicker penis for their partner, and those which add texture (e.g. ribbed outer sheath).”
To answer the rest of the questions we not only spoke to Ingber, but also to Jessie Cheung , M.D., a board-certified dermatologist who has vast experience treating men with various penis issues. Together, they helped to provide nearly everything there is to know about penis sheaths.
Most penis sleeves slide onto the penis like a condom, and function to enhance pleasure or improve functionality. “Penis sleeves were originally developed for medical purposes,” Cheung says. “Primarily, they were created to allow for penetration by men with erectile dysfunction, or to trap blood in the penis to maintain erections, or to reduce sensation for men with premature ejaculation.”
More recently, however, penis sleeves or extenders have become popular as sex toys . “They increase girth and length and can have nubs or ribs on the external or internal wall to provide stimulation for either partner,” Cheung says. So if your penis is significantly below average —remember the average erect penis is only 5.16 inches —a penis sleeve could be a helpful tool to add a few inches to your penis.
Yes, which partly explains why there are so many names for the device. There are penis sleeves for men who can’t get hard at all, and there are sleeves that work as long as you start erect and are then able to put on the device. These devices will help to keep you erect.
Then there are completely sheathed sleeves where your entire penis is covered. Other partially sheathed sleeves have an opening for the head of your penis, whereas other sleeves focus on adding girth to the base of your penis (for men who are blessed with length but want to add circumference). Then there are penis head sleeves if you wish to increase the size of your head. (So yes, there are options for every single shaped penis in existence!)
Last but not least, you have devices like the Elator and Erektor , which aren’t so much sleeves as they are extenders. Unlike sleeves, they don’t add artificial girth or length. These external penile devices have two fitted rings, one that goes around your penis base and the other around your head. There’s a rod that connects the two rings, keeping your penis erect and stretched out. It’s a potential option for men with mild to moderate ED.
A 2017 study published in Translational Andrology and Urology reported that some men who struggle with severe ED were actually able to reach orgasm while using a penis sleeve.
Penis sleeves are typically made out of silicone, though some are made out of rubber, latex, or plastic. “Make sure you don't have any sensitivities to the material being used,” Ingber says. “Several penis sleeves may contain latex, and many people have an allergy to this. Others are made from silicone, which is generally well-tolerated.” It’s also worth noting you can also buy a sleeve that vibrates, which can feel pleasurable for both you and your partner.
Yes. “A penis sleeve is a safe and effective alternative to surgery or medication for men with ED and is an easy tool to add variety to your sex life,” Cheung says. That said, penis sleeves and cock extenders are stop-gap measures. They don’t get to the root of whatever is causing your erectile dysfunction. They just help you in the moment if ED is something you struggle with. That’s enough for some men, but it may be helpful to speak to your doctor about other methods that help with ED long-term .
But do make sure to wash them regularly to avoid any infections, Ingber adds. “And like regular sex, be careful; having a penis sleeve does not protect you against things like STIs or injuries like penile fracture,” he says. So take it slow, use lots of lube, and enjoy yourself.
Yes, and we’ve written about them extensively , but Cheung provided three options she recommends for long-term size increase: penis filler injections, P-Shot injections, and penile laser treatments.
“Dermal fillers are not FDA-approved for use in the penis, but they can be safely injected by an experienced physician to increase girth, and can also increase sensitivity,” Cheung says. The P-Shot is a trademarked procedure that includes platelet-rich plasma injections to the penis and mechanical pumping to improve size and function. Stem cells and exosomes can be added to the classic P-Shot for greater tissue rejuvenation, Cheung adds. Last but not least, Cheung notes lasers are used to increase circulation and boost collagen, “giving lasting improvements in size and erectile function.”

Tempted by products that claim to increase penis size? Get the facts about what to expect from male-enhancement pills, pumps, exercises and surgeries.
Ads for penis-enlargement products and procedures are everywhere. A vast number of pumps, pills, weights, exercises and surgeries claim to increase the length and width of your penis.
However, there's little scientific support for nonsurgical methods to enlarge the penis. And no trusted medical organization endorses penis surgery for purely cosmetic reasons.
Most of the techniques you see advertised don't work. And some can damage your penis. Think twice before trying any of them.
The fear that your penis looks too small or is too small to satisfy your partner during sex is common. But most men who think their penises are too small actually have penises that are considered typical size.
Similarly, many men have an exaggerated idea of what is considered a "typical" penis size.
The length of a non-erect penis doesn't consistently predict length when the penis is erect. If your penis is about 5 inches (13 cm ) or longer when erect, it's of typical size.
A penis is considered small only if it measures less than 3 inches (about 7.5 centimeters) when erect. This is a condition called micropenis.
Advertisers would have you believe that your partner cares deeply about penis size. If you're concerned, talk to your partner.
Understanding your partner's needs and desires is more likely to improve your sexual relationship than changing the size of your penis.
Companies offer many types of nonsurgical penis-enlargement treatments. They often promote them with serious-looking ads that include support from "scientific" researchers.
If you look closely, you'll see that claims of how safe they are and how well they work haven't been proved. And these products may have potentially dangerous ingredients not listed on the label.
Marketers rely on ads with people who recommend the product. Marketers may also use data that's not accurate and questionable before-and-after photos. Dietary supplements don't require approval by the U.S. Food and Drug Administration. So manufacturers don't have to prove how safe a product is or that it works.
Most advertised penis-enlargement methods don't work. And some can cause permanent damage to your penis. Here are some of the most widely promoted products and techniques:
Vacuum pumps. Because pumps draw blood into the penis, making it swell, they're sometimes used to treat erectile dysfunction.
A vacuum pump can make a penis look larger temporarily. But using one too often or too long can damage elastic tissue in the penis, leading to less firm erections.
Exercises. Sometimes called jelqing, these exercises use a hand-over-hand motion to push blood from the base to the head of the penis.
Although this technique appears safer than other methods, there's no scientific proof it works. And it can lead to scar formation, pain and disfigurement.
Stretching. Stretching involves attaching a stretcher or extender device — also referred to as a penile traction device — to the penis for gentle tension.
A few small studies have reported length increases of half an inch to almost 2 inches (about 1 to 3 centimeters) with these devices. However, the activity may be uncomfortable. Also, it requires a commitment of at least 4 to 6 hours a day for many months to see results.
More research is needed to see if stretching is safe and if it works.
Studies of surgeries to lengthen or thicken the penis have found mixed results in safety, whether they work (effectiveness) and patient satisfaction.
At best, surgery may give a slight increase in girth to the penis. Or surgery may add a slight appearance of increased length to the non-erect penis. But it does not change the actual length of the penis. At worst, surgery can result in complications such as infection, scarring, and loss of sensation or function.
When the suspensory ligament is cut, the non-erect penis may look longer because more of it hangs outside the body. But it does not change the actual length of the penis.
The need for penis-enlargement surgery is rare. Surgery is typically reserved for men whose penises don't function because of a problem present at birth or an injury.
Some surgeons offer cosmetic penis enlargement using many techniques. However, it's controversial and considered by many experts to be unnecessary. And in some cases, it can be permanently harmful. These surgeries should be considered experimental. There aren't enough studies of penis-enlargement surgery to give an accurate picture of risks and benefits.
Examples of surgery done to increase the length or thickness of the penis include:
Cutting the suspensory ligament. The most widely used surgical procedure to lengthen the penis involves cutting the suspensory ligament that attaches the penis to the pubic bone. Also, skin is moved from the abdomen to the penis shaft. When this ligament is cut, the penis appears longer because more of it hangs down. But it doesn't change the actual length of the penis.
But cutting the suspensory ligament can cause an erect penis to be unstable. This lack of support during an erection could result in injury to the penis during sexual activity. Also, the ligament can grow back together, causing a shortened appearance of the penis. Cutting the suspensory ligament is sometimes combined with other procedures, such as removing extra fat over the pubic bone.
There's no guaranteed safe and proven way to enlarge your penis. But there are a few things you can do if you're concerned about your penis size.
Get in shape. If you're overweight and have a "beer gut," the extra belly fat may make your penis appear shorter than it is.
Regular exercise can make a big difference. Better physical conditioning may not only make you look and feel healthy, but also can improve strength and endurance during sex.
Many men feel better with reassurance that they are "typical." Advice about how to better satisfy their partner without resorting to cosmetic penis enlargement can also help.
Many men believe that increasing the size of their penises will make them a better lover or make them more attractive. But chances are your penis is within the typical size range.
Even if your penis is smaller than average, it may not matter to your partner. In addition, there's no proven way to make a penis larger.
The solution to your concerns about penis size may be as easy as talking with your partner or getting in shape. If those steps don't help, try talking with a professional counselor about your concerns.
