Whether there’s a lump or bump you’re worried about or you’re after a routine check-up to make sure everything’s in working order — if you haven’t had a genital examination before it’s normal to feel a bit nervous. Knowing what to expect during a male genital examination can help put your mind at ease.
A physical examination of your genital region — which includes your penis, scrotum, testes (testicles), anus and prostate — is vital for the detection of health conditions such as testicular cancer , Klinefelter syndrome , and penile and hormonal abnormalities. It’s also used for the diagnosis of hernia, urinary problems , skin conditions, and infection.
A genital examination will be done by your doctor. You’ll need to remove clothing covering the area. The examination may be carried out with you standing or lying down with a sheet or towel covering you. At any point you can ask questions or express any discomfort you’re feeling.
During the check-up, your doctor will assess the size and smoothness of your testes (testicles) and look for any abnormalities of your penis. They might also feel for scar tissue on the penis. They may also do a rectal examination of the prostate, in which they insert a lubricated, gloved finger into your anus.
If it’s suspected that you might have a sexually transmitted infection, your doctor will swab any skin lesion on the genital region or urethral discharge.
If you’re having issues with a tight foreskin, the doctor will examine the lack of mobility of your foreskin.
Overall, the process shouldn’t take more than a few minutes.
No, you don’t need to be erect for a genital examination. If you’re experiencing an issue that occurs during an erection, like bending , it can help to take a photo at home to show your doctor during your appointment.
Your genitals are incredibly sensitive, so it isn’t unusual for an erection to occur while you’re being physically examined. Don’t let it worry you, your doctor has seen it all before.

You can and should check yourself — getting familiar with the look and feel of your genitals can help you recognise when something has changed. However, it’s also important to get checked by a doctor as they’re trained to spot signs of problems you’re not aware of.
A testicular self-examination (TSE) is a quick and simple process. You might find it easier after a warm bath or shower when the skin of your scrotum is relaxed.
Standing in front of a mirror, start by checking for any swelling on the skin of your scrotum . Hold your scrotum in your hands and feel the size and weight of each testicle. Don’t worry if one testicle is a little bigger or hangs lower than the other, that’s normal.
Feel each testis, one at a time, by rolling it between your thumb and finger, checking for any lumps or swelling. The testes should feel firm, and the surface should feel smooth. You should be able to feel your epididymis, a soft tube toward the back of each testicle that carries sperm to the ejaculatory ducts. Check for any swelling in this area. You shouldn’t feel any pain when checking your testicles.
You should have a check-up every 12 months but see a doctor as soon as possible if you have noticed any changes to your genitals.
A genital examination should be part of a standard check-up for children and adolescents to check for normal development and puberty progression.

I was experiencing lower abdominal pains and I scheduled an appointment with my doctor. I found out he had retired so I made an appointment with the doctor who took over his practice. The doctor who took over the practice is female. I was waiting I the exam room for the doctor feeling a little anxious since I had never been examined by a woman. She came in and I told her about my stomach pains. She gave me the standard examination as well as checking my lower abdomen. The last thing she did was the hernia check. She asked me take off my pants. I dropped my pants and just stood there because I wasn’t sure she wanted me to pull down my underwear. After a few seconds of her staring down at my crotch she got down on one knee and attempted to perform the hernia check though the leg opening in my underwear. When she realized that wasn’t going to work , she put her hands on the waistband of my underwear and slowly started to pull them down. This made me feel a little uncomfortable so I quickly pulled down my underwear exposing my erect penis to her. My erection was only inches from her face and what made it even more embarrassing is that my penis is only 4” fully erect. She then performed the hernia check (turn and cough, turn and cough). I was afraid to look down. When she finished, I quickly pulled up my pants and noticed that she was looking at my erect penis. She started to write on my chart and ask me some additional questions. I guess I was still a little freaked about her seeing my erection because I was standing up instead of sitting down on the exam table. She must have noticed because she told me I could sit down. When it was time for me to leave the exam room, I thanked her and she looked me right in the eye and smirked and smiled. I am a doctor myself, a 39 years old male, employed in a hospital. Regulations impose a regular medical check-up once a year on all members of the medical personnel, including doctors and nurses. Examinations are always thorough to protect public health. Previously, I have had to complete some tests including blood count, blood lipids, urine, HIV and HCV testing as well as ECG, ECHO and chest X-ray. I appeared in the occupational physician office on scheduled time, at 7.30 in the morning. The doctor was a tall, very attractive woman in her early thirties, dressed in a white above-the-knee tunic, nude ultra-sheer stockings with open toe sandals. Her toes and fingers were polished in bright red. Her leg were perfectly shaped and skin smooth. The day was warm, apparently she had nothing under her tunic except of white sport bra and a thong. Her body was well toned, clearly she used to spend her free time in a gym. She smiled, shook my hand and took detailed medical history. Then I was given a paper gown and a single use g-string and told to change behind a curtain. Though I was a medical professional, I was always a bit embarrassed being a patient myself. Then I was weighed and measured (BMI), my blood pressure and pulse were taken. The doctor examined my head and neck, oral cavity, teeth, thyroid gland and palpated for major lymph nodes, including underarms. My armpits were dry and clean, as I have previously performed laser hair removal. Then she listened to my lungs and heart and palpated abdomen. Finally she asked me to disrobe entirely and to stand at the end of the exam table with feet about shoulder width. She put on blue latex gloves saying “ I will examine your penis, scrotum, and testes”. She gently grasped shaft of my penis and inspected glans for any abnormalities and hygiene. I was circumcised high and tight with a well defined scar, thus my penis was in perfect condition. “I advice all men to be cut in this way. It's very aesthetic and prevents from masturbation” she said while examining my genitals. The doctor was sitting before me on a rolling stool, as her stockinged red toes were close to my bare feet. I was not surprised when my member started to grow. “Don't worry, it's a natural response. Forget, you're a doctor here, You're just a healthy male” she assured moving to my scrotum and examined all sides of the sac, lifted it up to visually inspect the posterior side and examined the scrotal sac by rolling the skin between the fingers of one or both hands. Then she said: "I am going to examine your anus, rectum, and the prostate. I will be inserting my finger into the rectum. You should feel some pressure, but if you feel any other discomfort; please let me know. " I bent over with arms resting on table, feet slightly separated, and one slightly flexed. She generously lubricated the index finger of the gloved dominant hand with lubricant, separated my buttocks with two hands and visually inspected the area for lesions, rashes, fissures and hemorrhoids. She placed her index finger at the anal opening. "To make the insertion of my finger into the rectum more comfortable, please bear down as though you were going to have a bowel movement. You will only feel pressure " she instructed and applied gentle pressure and slowly inserted finger in downward angle towards umbilicus allowing a few seconds for the external and internal sphincter to relax. The doctor evaluated the tone of the sphincter muscles and rotated finger 360° to evaluate anal sphincter muscle ring. I was hard as a rock in this time, but she didn't pay any attention. To examine the prostate, she positioned finger palmar surface down and palpated in a circular motion to increase ability to identify the lobes and groove. Prostate massage resulted with massive leakage of pre-seminal fluid, she collected into a tube. She didn't stop tu rub my gland, all my muscles tensed and I was not far from ejaculation. Fortunately, the exam was over. I wiped myself with a paper towel and dressed up. “Your health and physiological reactions are perfect. Fit to work hard” she concluded.

I agree but I think she enjoyed it.
