There is so little information presented that is actually thoughtful and/or original. You could get more useful info reading the discussion pages for the topic in Wikipedia. 70 pages of alternates for "Two in the pink...." with 5 alternates per page to stretch the page count.

The index and middle fingers are kept together (touching) and the back of the hand faces outwards (away from the gesturer). The gesture refers to the act of inserting the index and middle fingers into a vagina and the little finger into the recipient's anus, hence the "shock".
