In my state meds can only be forced if the patient is a safety risk to self or others, or if there's a specific court order to give the meds even if involuntarily. The court order hospitalizing the patient isn't enough to allow meds to be forced.

The nurse can refuse the assignment for any of the above reasons LEGALLY in order to protect the patient and his/her license. However, the hospital can still fire the nurse for insubordination - LEGALLY in a right-to-work state. The nurse can sue the hospital and demand to be reinstated. Those cases are decided on a case-by-case basis.
That's pretty harsh to write up a CNA because the patient refused a bed bath. Patients have the right to refuse meds and treatment, but nurses and doctor's don't get written up. So why should a CNA get written up for a patient refusing to be washed?
