My girlfriend is a nurse practioner student and I’m struggling to get used to a few things. When she exam on the teaching patient and told me about how she palpated the penis it was a bit awkward. All seems a bit strange. I have never had a doctor (yes mine is female) who brought a chaperone into a genital exam.

Yeah that was completely inappropriate. I would not have allowed the doctor to remove the drape with the receptionist in the room. If the doctor tried to insist, I would have asked them both to leave while I got dressed and then I would have left.

This was improper it violate Code of Medical Ethics Opinion 3.1.2 which reads in part: "When individuals who are not involved in providing care seek to observe patient-physician encounters, e.g. for educational purposes, physicians should safeguard patient privacy by permitting such observers to be present during a clinical encounters only when:
(a) the patient has explicitly agreed to the presence of the observers(s)."
The doctor should have asked for consent to have anyone else in the room watching a modestly sensitive examination.

What is wrong here is not so much that the female doctor used her secretary but that she did not ask you first before using any other female as a chaperone.
Code of Medical Ethics Opinion 3.1.2 reads as follows in party:
"Individuals legitimately present during patient-physician encounters include those directly involved in the patient’s care, and can include other members of the health care team or employees of pharmaceutical or medical device companies when they are present to provide technical assistance, in keeping with ethics guidance.

When individuals who are not involved in providing care seek to observe patient-physician encounters, e.g., for educational purposes, physicians should safeguard patient privacy by permitting such observers to be present during a clinical encounter only when:

The patient has explicitly agreed to the presence of the observer(s)."

The secretary was not directly involved in patient care and had no business being there without your explicit consent. Nor would the nurse without your consent since she would not have been directly involved in your medical care. !

I suppose I've always been asked but I never really paid much attention and don't really care. At the male physician's office who my wife and I both had until Obama care became law and he closed his doors (you can keep your doctor). He worked alone and had a female receptionist. He never bothered to have anyone come in if we were going to be naked for an exam. We were always naked for a physical. She might bring in something he needed on occasion. With my new female doctor I'm never naked anyway and a "physical" only amounts to hi how are ya listening with a stethoscope over your clothing and any questions? If you want the doctor to see your skin you have to make an appointment with another doctor (more money). I guess that's how they get around the reduced payments of Obamacare.At the VA she has no witnesses either. Once she had a female trainee examine me. She did ask me if that was OK. She did the most thorough exam I've had in years. They do coordinate the blood tests so I only have to get one drawing.
We're all adults, we know what the other gender looks like, what's the big deal?

Yes, it is OK. the physician can use a chaperone if she wants. What difference does it make if it is a medical assistant or a secretary? It is not like medical personnel get lessons in seeing naked patients. What should have happened is that your physician should have asked you first. It is about communicating with your patients! A similar thing happened to me too. If it happens again I will get up and walk out.

As for the secretary winking at you that is inappropriate. I personally would take it as a compliment. Would you feel any different if the secretary was gay.

I personally would not go back to that physician. There really is not need for a female physician to use a chaperone with a male patient. Men are not passively aggressive. They will not complain that they were sexually abused when they were not. In any event it was very rude of your physician not to ask for your consent before bringing in a female chaperone. The rule is that when you go to a physician you obviously consent to her examination. However, you only consent to what you consented to and what you consented to was whoever provided you care. You never consented to anyone who was not necessary to that examination.

The use of Chaperones is covered in AMA Code of Medical Ethics Opinion 1.2.4 and required that the policy on chaperones be communicated to the patient. Patient Privacy and Outside Observers to the clinical encounters is covered by AMA Code of Medical Ethics Opinion 3.1.2 and requires that the patient explicitly agree to the presence of observers. Your objection should not be that a female secretary was present but that ANYONE was present to sit and watch without your permission or even being informed in advance!
