With his hood up and plastic bags wrapped around his feet, a man of indeterminate age staggered out of a dirty alley in Vancouver, British Columbia. He lay down on a piece of sidewalk a few meters away, soaked by the continuous rains of early January. His eyes vacant, he waited. Within a few hours, the craving would propel him to his feet and he would set out in search of a few grams of heroin or fentanyl. After overwhelming the United States, this synthetic opioid, a hundred times more powerful than morphine, has become the most widespread drug in Canada since the turn of the century. Just a few steps away from the gleaming buildings of Vancouver's business district — and not far from the highly touristic and gentrified Gastown district and the local Chinatown — the streets of Downtown Eastside are covered with makeshift tents in the form of worn-out tarps hung on the facades of decrepit buildings. Most of the businesses have deserted these few blocks, nicknamed 'Zombieland' across the country. The only inhabitants roaming around here are other ghost-like figures resembling the man in the hood. Most of them are men — women remain mostly hiddden — living with some combination of poverty, mental health problems and addictions. For a few dozen dollars a day, they buy the doses that will help them make it through to tomorrow but will also almost kill them. This neighborhood, and the province of British Columbia BC more broadly, has long struggled with the ravages of drug use. The heroin and cocaine trade that exploded in the mids was followed by crack cocaine and synthetic opioids such as fentanyl, which are cheap to produce and are particularly lethal. As early as , BC declared a state of health emergency across the province — overdoses have since caused the deaths of 14, of its residents, or more than half of all drug victims in Canada. On the country's west coast, the epicenter of the crisis, this silent epidemic has claimed almost as many lives as SARS-CoV Over the past two years, provincial police departments have counted 4, overdose deaths, compared to 5, total deaths from the Covid pandemic. In an effort to curb this plague, BC obtained from the federal authorities, in May , an exemption to the law on narcotics, in order to decriminalize drug consumption.
