Alright, buckle up Witcher fans - your ultimate wish has finally been granted. Assuming that your ultimate wish in life is to have a nude Geralt of Rivia following you around Skyrim, which sounds like a pretty reasonable assumption.
Skyrim's modding community is one of the biggest, most active (and creative) out there, with a good 60,000 mods for the game currently listed on the Nexus Mods site. Amongst them you'll find a bit of everything - the awesome (like adding Morrowind creatures to your game), the funny (like replacing mudcrabs with Futurama's Dr Zoiberg) the wholesome (like giving backpacks to your dogs), and the...not-so-wholesome. Like what we have here.
One of the latest additions to Skyrim's adult mod collection is called the 'Unique SOS body type and skin textures for Geralt follower', and comes courtesy of modder matthouss. The mod literally adds a very nude Geralt to your game (as you might imagine, the Nexus Mods page in question is significantly NSFW, so search for it at your own risk), but in order for it to work you need to have a few prerequisite mods in place first.
First off, you'll need the Geralt of Rivia voiced follower mod by bchick3, which lets you take on the ol' White Wolf as your very own companion. Then you also need to install the not-very-subtly-titled adult mod, SOS: Schlongs of Skyrim.
Matthouss proudly states that they have made a unique body mesh for Naked Geralt that accurately reflects the scars, rippling muscles, and other relevant - ahem - details of Geralt's physique. They also kindly included some adjustments to his hair, beard, and eyes for good measure.
Note that this mod is only available for Skyrim Special Edition at this point, though. If that's not the version you have, you do have a vanilla Oldrim version of the Geralt of Rivia voiced follower available to you, but no compatible version of the Nude Geralt mod. Not to worry, though, there is hardly a lack of comparably NSFW mods for Oldrim at your fingertips.
Roll your eyes, grind your teeth, voice your complaints, but you know as well as I do - we'll be in Skyrim come November.
