Dr. Joel Kaplan developed different cylinder sizes to maintain a good vacuum seal while allowing enough room for maximum growth. Instead of using a “one size fits all” approach that is less effective, Dr. Kaplan decided to use the best options that will allow for faster and better growth.
Wrap a tape measure around the base of your penis while erect. If you do not have a tape measure, use a string to wrap around your penis and mark with a pen where the string intersects.
Take the string off of your penis. Line it up next to a ruler and mark the measurement in inches. This is your base circumference measurement.
Sometimes the penis will be bigger at the penis head than at the base of the shaft. To make sure the cylinder will be comfortable, it is recommended to measure the circumference of the penis head. Wrap another string around the thickest part of the penis head, and mark where the string meets.
Take the string off of your penis. Line it up next to a ruler and mark the measurement in inches. This is your penis head circumference measurement.
Our standard cylinders are 9” in length. In order to make sure you have room to grow, we need your erect penis length. Take a ruler, and line it up against your erect penis. Push the ruler a little bit against your stomach (about 1/2 inch). Measure your full length to the top of the penis head. That is your erect penis length.

In a recent video on the UroChannel on YouTube, Dr. Stefan Buntrock, a urologist and sexologist, clears up some questions and misconceptions surrounding penis size and how to measure a penis accurately, starting with: do you do it while the penis is flaccid, or erect?
"Preferably both, also with the phenomenon of growers and showers in mind," says Buntrock. "However, the erect size can be estimated by stretching the flaccid penis. This is done standing, with the penis parallel to the floor, at an angle of 90 degrees. If done correctly, the stretched length comes pretty close to the erect length, but it is necessary to apply a certain force."
Another common question is where, exactly, you should start measuring from, and on which side. Buntrock explains that you measure starting from the base to the tip on the top side, or dorsal side, of the penis. "Body composition is a confounder," he adds. "The more body fat, the smaller the penis appears... Therefore, I prefer to measure from the pubic bone." He also recommends using a tape measure rather than a rigid ruler, due to the slight curvature of the penis.
He goes on to say that there are several factors which can make it difficult to measure size, such as temperature, with colder environments often decreasing the size. Even the act of having one's penis measured can cause anxiety, leading to the contraction of blood vessels, making the penis smaller.
