vaginal delivery of a baby.
Delivery is also known as the second stage of labor,
or part of the second stage of labor. It begins with complete
dilatation and ends when the baby is completely out of the mother. The
exact time of delivery is normally taken at the moment the baby's
anterior shoulder (the shoulder delivering closest to the mother's
pubic bone) is out.
As the fetal head passes through the birth canal, it
normally demonstrates, in sequence, the " cardinal
movements of labor ." These include:
Engagement (fetal head reaches 0
station.)
Descent (fetal head descends past 0
station.)
Flexion (head is flexed with the
chin to its' chest.)
Internal Rotation (head rotates
from occiput transverse to occiput anterior.)
Extension (head extends with
crowning, passing through the vulva.)
External Rotation (head returns to
its' occiput transverse orientation)
Expulsion (shoulders and torso of
the baby are delivered.)
As the fetal head descends below 0 station, the mother
will perceive a sensation of pressure in the rectal area, similar to
the sensation of an imminent bowel movement. At this time she will
feel the urge to bear down, holding her breath and performing a
Valsalva, to try to expel the baby. This is called "pushing." The
maternal pushing efforts assist in speeding the delivery.
For women having their first baby, the second stage
will typically take an hour or two.
Delivery of the baby
During the delivery, the fetal head emerges through the vaginal
opening, usually facing toward the woman's rectum.
As the fetal head delivers, support the perineum to
reduce the risk of perineal laceration from uncontrolled, rapid
delivery.
After the fetal head delivers, allow time for the
fetal shoulders to rotate and descend through the birth canal. This
pause also allows the birth canal to squeeze the fetal chest, forcing
amniotic fluid out of the baby's nose and mouth.
After a reasonable pause (15-30 seconds), have the
woman bear down again, delivering the shoulders and torso of the baby.

Earlier this week, Lil Nas X tweeted that the video for his new single "Industry Baby" was "NOT FOR YOUR KIDS." And boy, he wasn't kidding. If this is what life in the clinker is like… well, lock us up.
The record-breaking artist (and EW cover star ) first teased the song , produced by Kanye West and Take a Daytrip and guest-starring rapper Jack Harlow, with a two-minute clip of a mock Supreme Court case that spoofed his recent Satan shoes fiasco with Nike and featured him in multiple roles: his attorney, the prosecutor, the judge, a tsk-tsking jury member, and Nas himself, taking the stand to deliver his defense. The trial culminated with him being sentenced to five years in Montero State Prison not for his clash with the sneaker giant, but for being gay.
Now, in the music video, which landed at midnight Friday, Nas returns to troll his doubters and homophobic haters even harder. He spanks and throws cash at his twerking cellmate, claims a pair of lovers for his internment, and — in the most jaw-dropping sequence — dances wet and buck-freaking-nekkid in the jailhouse shower with his fellow nude male prisoners.
"I told you long ago on the road/I got what they waitin' for," he sings, after polishing up his Grammys and smugly straightening his framed platinum record in his cell. "Ion' run from nothin', dawg/Get your soldiers/Just tell 'em I ain't layin' low."
Think the chart-topping "Old Town Road" singer would wind up a one-hit wonder? Think again. Nas claps back at anyone who's deemed him such, boasting, "Funny how you said it was the end/Then I went, did it again." He also goes on to spit a retort at homophobes: "I don't f--- b----es, I'm queer/But these n---- b----es like Madea."
On top of that, "Industry Baby" features a cameo from Teen Wolf and Arrow actor Colton Haynes , who plays a security guard lusting after Lil Nas X's "Montero (Call Me by Your Name)" video on his phone. Sporting a hot pink uniform and white kicks splashed with a rainbow stripe, Nas swoops in, catches the hypocrite in the act, and slugs him. The saga concludes Shawshank Redemption -style, with Nas revealing a hole he's carved into the wall of his cell and busting out of captivity to begin his new life.
Sexy slammer shenanigans and prison breaks aside, some of the lyrics reflect a letter the artist penned to his younger, closeted self ahead of the song's release.
"I wrote a song for us," he began in a tweet he posted Wednesday. "I know sometimes you feel like it's all downhill from here. I know your sexuality has made you feel like an outcast amongst your peers. I know going from having the biggest song in the world to being trapped in your apartment is weighing heavy on you. And I know if you hear the phrase 'one hit wonder' one more time you might combust. But I need you to keep going."
He continued, "I need you to realize that you have the opportunity to be the person that you needed growing up. I need you to stop feeling sorry for yourself. And I need you to remember that the only person who has to believe in YOU is YOU."
Lil Nas X's previous video for "Montero (Call Me by Your Name)" sparked the ire of religious conservatives and homophobes alike, depicting the performer pole-dancing straight to hell to give the devil a lap dance before killing him, taking his horns, and claiming the throne for himself. After the fallout, he was quick to respond to his dissenters on social media, often in playful and sharp-tongued ways.
"At first I was really afraid of alienating any of my straight fans," he previously told EW . "But then it was kind of like, if they feel offended, they were never really here for me. They were here for whatever version of myself they made up in their head."
"Montero," " Sun Goes Down ," and now "Industry Baby" will be available on Lil Nas X's debut album, Montero , which he said in June would be "coming soon." If certain lyrics in "Industry Baby" are any indication — "Uh need to get this album done/Need a couple number ones/Need a plaque on every song/Need me like one with Nicki now" — we just might be getting a Nicki Minaj collab too.

Basal thermometers. Ovulation predictor kits . Cervical mucus. Are you hot yet? If not, you're not alone. While babymaking sex sounds great in theory (no worrying about birth control!), couples who've been there are more likely to describe it as "mechanical," "unromantic," and "a big turnoff."


What's the problem? One husband explains, "When we threw away the condoms, I thought that meant we'd get to have spontaneous, uninhibited sex. Instead it was just the opposite. We had to schedule intercourse to correspond with my wife's ovulation, and limit ourselves to a couple of positions because they offered maximum penetration."


The experience can be equally frustrating for women. "I wanted it to be a beautiful thing—that we loved each other enough to create a baby together," says one mom-to-be. "But the reality was more like some weird science experiment."


According to psychotherapist Lenore Pomerance, the key to a better sex life is learning to separate fertility from sexuality. "There's a vast difference between sex for pleasure and sex for conception, and couples who recognize this can avoid blaming, feeling guilty, or getting turned off by sex for conception ," she says.


Pomerance points out that this separation is most essential when your efforts aren't immediately successful. "I've had couples express fears that they couldn't get pregnant because the sex wasn't good enough, or the love failed—which, of course, has nothing to do with fertility. When partners start feeling flawed or inadequate because they haven't conceived right away, they grow depressed and their sexual feelings are affected."


1. Start learning about "reality sex." Pomerance says, "Couples who are disappointed by babymaking sex are often hanging on to a fantasy about what lovemaking is 'supposed' to be—romantic, spontaneous, always satisfying, and not requiring communication or effort to get or stay that way. The reality is that sex can be many, many things. If a couple is connected enough emotionally and physically, they can let go of the fantasy and learn how to make the reality feel good."


2. Keep your sense of humor. Viewed objectively, sex, conception, and becoming a parent are three of the funniest conditions in the human experience. Couples who can laugh at themselves often end up closer, sexually and otherwise, than they were before trying to conceive. One new mom who had to undergo two years of fertility treatments says that laughter is what got her through. "In spite of all our best efforts, we ended up having to conceive by intrauterine insemination. If we hadn't been able to joke about the 'turkey baster,' we'd probably be divorced now instead of expecting our second child."


3. Have hot, spontaneous sex the rest of the month. By definition, you can only have babymaking sex during the three- to five-day window when you're fertile each month . That leaves nearly four weeks for recreational fun! Try to get in some tender lovemaking as well as swinging from the chandelier. And take time to share your feelings about your sexual connection. Couples who start working to keep it healthy now reap big benefits when they become parents.
