It's often the case that people use ortho in areas they fly often, and the HD mesh scenery outside of those areas. It means that if you compare meshes generated by Ortho4XP and HD mesh v4, you will find that the mesh from HD Mesh v4 has both worse performance and less detail than a mesh from Ortho4XP. I'm not sure why Alpilotx chose this approach for the latest HD mesh, but there's probably a reason that's beyond me.
