هناخد انهارده اي حلوين؟!

هناخد ال forearm fractures

متخافش جزئية الاناتومي اللي محتاجينها مش كتيره خالص

عارفين ان forearm بيكون من عظمتين ال ulna(medial) و radius (lateral) و دول مع بعض بيعملوا 2 joints من فوق ال ulna مع trochlea بيعمل elbow joint و من تحت ال radius بيعمل مع metacarpals و يكونوا ال wrist joint

وulna و radius بين بعض بيعملوا 2joints من فوق superior interosseous joint و من تحت inferior interosseous joint و بينهم فيه interosseous membrane بعدي من قدام Ant.devision of median nerve و من الخلف post.devision of median nerve

                                                          :1epidemiology

بيقولك ان open fracture of forearm نسبته أعلى من closed لأي fracture تانيه ماعدا ال tibia

M>F و غالبا بيحصل بسبب حادثة او سقوط او العاب عنيفة وكدا..

                                                Mechanism of injury :

متعودين لاما direct او indirect بس هنا فالنوع دا ال direct هو الأغلب direct>indirect

                                                                 3pathology:

هنا بيتقسم لاتنين ع حسب انهي bone اللي حصلها trauma

                                                           1- ulna fracture

و دي أكثر عظمة بتتكسر لأنها أضعف من radius وليها classification اسمه monteggia classification

كل ال 4 types بيشتركوا ان حصل fracture of upper 1/3 of ulna لكن بيختلفوا ع اتجاه ال dislocation of radius

Type 1: fracture of upper 1/3 of ulna + anterior dislocation of radius

Type2: fracture of upper 1/3 of ulna + posterior dislocation of radius

Type 3: fracture of upper 1 /3 of ulna + lateral dislocation of radius

Type 4: fracture of upper 1/3 of ulna & radius + anterior dislocation of radius

هنا الكسر حصل ف ulna و radius و كمان حصل Ant dislocation لل radius

radial fracture 

و دا غالبا بيكون indirect وليها تقسيمة خاص بيها اسمها galezzi classification

ودا هنا حصل fracture of lower 1/3 of radius و معاه dislocation of inferior radioulnar joint بيتقسم الي

Type 1: posterior displacement of shaft of radius + anterior displacement of head of ulna

و دا ال lower 1/3 of radius اتكسر و حصله post.displacement و ال head of ulna حصلها العكس Ant displacement

Type2: anterior displacement of radius + posterior displacement of head of ulna

ودا عكس type1

clinical picture

نفس الاسطنبة pain, swelling, deformity, loss of function

ولازم اعمل NV examination كويس علشان ال radial&ulnar nerves

واخلي بالي لو حصل compartment syndrome و دا بسبب فيه hematoma مثلا جوه العضلات و الضغط عالي جدا بس ال fascia اللي محوطة العضلات قوية فحابسه الضغط النتيجة المتوقعه ان هيحصل damage of muscles & blood supply طب ودا أشك فيه امتى؟

لو عندي unremitting pain with passive stretch of digits و علاجه افتح بسرعه الfascia دي بعمليه اسمه fasciotomy.

فيه complication ممكن تحصل اسمها synostosis دي يعني ال proxiaml fragmentof radius يلحم مع distal fragment of ulna و العكس

                                                            5-Radiograph

بعمل x ray ب 2views اللي هما AP & lateral ولازم اعمل x ray ع ال elbow و wrist

                                                             6- treatment

نفس الاسطنبة لو undisplaced يبقى closed reduction & external fixation

اما لو displaced يبقى ORIF

بس اخلي بالي فالاطفال بالاخص في حالة radial fracture ان ال rotational deformity متزدش عن 10degree علشان ميحصلش مشاكل للعظم أثناء ال growth

طب ال rotational deformity دي بتحصل ازاي؟؟

ع حسب الكسر كان proximal to pronator teres و كدا هيحصل ولا distal ليها فكدا مش هيحصل pronation و هاحط صورة توضح

هنكمل جزئية صغيرة وهي distal radius fracture

ودي تعتبر most common fracture of upper limb كللله نركز!!

و بتيجي غالبا في old age و خاصة postmenopausal females علشان ال osteopetrosis زي مين ي شطااار؟؟!

ايوه بالظبط زي ال proximal humerus

يعني اول حته من ال arm و آخر حته من forearm علشان نفتكر

و هي فيه منها ٤ انواع colles, smith, barton, chauffeur

هتاخد كل واحدة و نشرحها يلا بسم الله

اول نوع colles fracture و دا أشهر نوع

Mechanism of injury: fall on out-stretched hand with extended wrist

                                                      Pathology:

دلوقتي انا وقعت ع ايدي نفسها و هي مفرودة اتجاه ال trauma من قدام (palm) للخلف ( dorsum) فال distal part of radius هتتكسر و هيحصلها posterior displacement

و كمان forward و يحصلها impaction

و كمان lateral displacement

يعني كدا حصلها 3displacements

اما ال proximal part هيحصلها pronation بعضلات ال pronators و هتاخد شكل dinner fork deformity ودا شكل مميز جدا لل colles و ممكن يحصل sublaxation او displacement لل inferior radioulnar joint

طب اي ال C/P : نفس الاسطنبة + dinner fork deformity

طب اي ال complications : نفس الاسطنبة + radial artery injury & median nerve injury و بسبب ال swelling و ال edema اللي بتحصل ممكن يحصل fibrosis في carpal tunnel و يضغط ع median nerve و يعمل carpal tunnel syndrome

ولو الكسر دا جه ف طفل ف physis كدا ال radius مش هتطول و ال ulna هتطول وتنمو عادي و هيسبب madlung deformity

نركز! كدا ذكرنا 2deformities واحد بيحصل بسبب الكسر نفسه وهووdinner fork و التاني بيحصل في ال children بسبب توقف النمو ف ال radius و دا هو madlung

forward>>>>> هاعمل traction & counter traction

Posterior>>>> هاعمل anterior reduction

Lateral(radial) >>>>> هاعمل medial reduction

طب هاعمل ال fixation في انهي وضع؟

Forearm pronated, hand flexed, ulnar deviation

يعني عكس ال trauma بثبته ب below elbow cast for 6 weeks

النوع التاني smith fracture او reversed colles

ودا عكس ال colles

Etiology : fall on out-stretched hand with flexed wrist

Pathology:fracture of distal radius with anterior displacement

و العلاج عكس ال colles علشان منطولش

النوع التالت barton fracture و دا حصل siatal radial fracture برضو بس حصل معاه intra articular fracture سواء حصل معاه anterior or posterior displacement بس المهم ان حصل كسر ف المفصل intra articular

النوع الرابع chauffeur او radial styloid fracture

ودا بيحصل لو سواق مثلا عمل حادثة فحصل avulsion fracture نتيجة لل compression of scaphoid against styloid

و اسمه chauffeur ع اساس انها بتحصل للسواقين اكتر حاجة

تقدر تاخد بريك بس اتغدى كويس وكل فراخ لان لا قهوه ولا نوم هيفوق للي جاي و نحبس بشاي و حصلتي ف التيليجراف اللي جاي 🚴‍♂️
