Serbia and Montenegro is situated in southeastern Europe, on the Balkan Peninsula. Its geographic position makes the country one of the major illicit drug routes for drugs, especially cannabinoids and heroin coming from the Middle East to Central and Western Europe. Regarding legal drug use, it should be mentioned that both tobacco smoking and alcohol drinking are widely spread, have the roots in tradition, and are not stigmatized at all. Daily consumption of tobacco in the country is 53 tons about 70 million cigarettes. As for illicit drug use, there is significant lack of research and statistical data, and researchers can only look to some estimates published in the literature. The estimation is that there are about , drug addicts in the country, of which 30, are heroin users. Several reports have been made that drug abuse starts as early as in primary school age 13 to15 years old. Regarding heroin use, there are reports that the mean age of first use is 16,2, and the mean age of first intravenous use is 18,2. But these reports are obtained mostly from the small sample, local research. Unfortunately, there was no national survey, so there is a serious lack of data. There is a noticeable trend of increasing substance abuse and addiction, and the most commonly used drugs are cannabinoids, heroin, anxiolytics, and methylenedioxymethamphetamine MDMA or Ecstasy. This increasing trend is noticed by the increase of patients over time. The National Institute for Addictive Diseases has been regularly admitting addicted patients for treatment since , with patients per year on the average. Since this number increased to and reached in Besides the above-mentioned institution, there is no other facility specializing in the treatment of drug abuse. Several psychiatric hospitals all over the country have departments for treatment of this profile of patients, but some have an insufficient number of beds or just offer ambulatory treatment. What is most commonly offered is ambulatory or inpatient detoxification with vitamins, glucose, and symptomatic therapy, while some private facilities offer detoxification with the use of antagonists for heroin users. State clinics are mostly in poor condition, with the serious lack of equipment and medications. There is no structured aftercare provided in the country. Overall, there is a substantial need for research in order to learn the real situation in the country, as well as for development of more treatment and aftercare modalities and more capacities that would deal with substance abuse and addiction.
