A man(in his childhood at the age of 17 years) had sex via docking as head of penis to head of penis of another man(aged of 17 years) and ejaculated recieved by his opening on penis head 16 years ago,the HIV status of another man was not known.
What is the risk for HIV transmission in this case and what can be the maximum duration of asymptomatic period in HIV infection ?
The risk of docking, i.e, covering a partner penis with one's foreskin and rubbing the two together, is considered to be a negligible risk for HIV transmission. In order for HIV to be transmitted, a detectable amount of the virus must have a direct route from one person's body into the mucous membranes of another. Although theoretically docking could provide a means by which the ejaculate from one penis could be directed into another, it is still extremely unlikely, and so far unheard of. After 36 years and over 70 million HIV acquisitions, there has yet to be a single case reported of someone receiving HIV in this way.
As far as asymptomatic periods, there simply is no one answer to this. Some people can live decades with HIV and remain asymptomatic. Some people begin to feel unwell within weeks. What's crucially different now from 16 years ago is the understanding of how the HIV medicines work. Back then, if someone newly tested HIV positive, many doctors took a "wait and see" approach before starting someone on meds. Now, given how effectively and quickly the current drugs work, most doctors will prescribe treatment medications almost immediately upon an initial HIV diagnosis. We now know the sooner the medicines are taken, the better they work.
You don't mention how many, if any, HIV tests the man in question has received over the past 16 years. If he hasn't had one, then now would most certainly be the best time. Given the docking activity in question carries no inherent risks, wouldn't it be wise for this person's peace of mind to simply test and stop living with this unknown question mark hanging over his head?
