People do not wear diapers in any of the mental health institutions or other residential health facilities I have worked in or visited.
Mental health issues in and of themselves do not cause incontinence. The inability to control bowel and bladder may be associated with spinal cord injury, brain injury or certain age-related dementias. Some persons suffering from mental illness may not use the toilet appropriately, but there are other ways rather than diapers to deal with such behavior.
The nursing care facilities I worked in approached incontinence by frequently bringing patients to the bathroom, and using absorbent pads on the bed for accidents. I never saw a diaper used on an adult patient.

I have several diaper humiliation fantasies so I thought I would write them here. this one is about needing to be diapered while in the hospital. I had just woken up from the surgery when my nurse came in to check my vitals. She told me the surgery went well and that I should be able to leave the next day since it wasnt a very major surgery. I panicked at first, my mind was scrambling to find a way to hide my embarrassing accident from the nurse, but then she walked in the room and saw it. "Uh oh, sweetie don't worry, I will get your bedding changed". I couldn't help but blush as she helped me to a chair and handed me clean clothes to change into while she cleaned my bed. I had just gotten my dry clothes on when we heard a small splash hit the floor. I thought I had knocked over my water bottle, but looked down to discover I had peed myself again. I burst out in tears and the nurse tried to comfort me. I told her I didn't know what was happening and I didn't feel the urge to go either time. "Don't worry honey", she said sweetly, "I will get something to help you out until the doctor can see you." I got back into the bed and waited for her to return. When she came back she had what looked like a bunch of small white pillows. As she got closer I realized they were diapers. My face turned bright red, and I started to cry. "Oh, sweetheart, don't cry, plenty of adults need protection, besides it's just until the doctor can see you and figure out what's going on." I just lay there as she pulled the covers down and prepared to diaper me. My face was red as she stripped off my panties and unfolded a diaper before sliding it under me. "Oh god" I thought to myself as my bottom touched the soft material, " this thing is so thick!". She then pulled the front up over me and taped it closed. I couldn't believe I had just been diapered for the first time since I was 2. I couldn't stop blushing. The nurse left the room and I decided to get some sleep while waiting for the doctor. It was hard to get comfortable due to the thickness of the diaper. I felt like a huge baby. A few hours later I was woken up by a male voice. It was the doctor. "Hi, I'm Dr. Smith. I'm here to see what's going on with you." I sat up in the bed and realized he wasn't alone. There were about 5 or 6 nurses with him, all male except one. He saw the confused look on my face and explained, "these are my students, they help me with all my patients." I nodded my head and the young nurses circled the bed. "So your nurse tells me you are in diapers due to sudden incontinence after your surgery.." My cheeks burned at his words and a couple of the young men snickered. All I could do was nod my head. "Well let's have a look and see what's going on." He pulled my blankets back exposing my very wet diaper. I had wet myself in my sleep! I closed my eyes in embarrassment and the doctor said, "well, we need to do a physical exam of your urethra to be sure its not damaged, but first we need to get you out of this wet didee!" Tears ran down my rosey cheeks as one of the hott young men was instructed to powder my tushy and my "little girl parts" as the doctor said. The nurse gave me a generous dusting of powder on my front and the others lifted my legs behind my head so the hott nurse could powder my fat hiney. After he rubbed the powder in they settled my bottom back on the open diaper. Just then there was a knock at the door. "Come in!", the doctor yelled. I froze as my family walked in. I wanted to crawl in a hole and die! My parents and my brother and sister, and their girlfriend and boyfriend all walked in. They were going to witness the end of my diaper change! I was laying there on an open diaper with a freshly powdered butt and my legs spread eagle. Could it get any more embarrassing?? It didn't help that my sisters boyfriend was my ex. He had the biggest grin on his face. Things had ended badly between us so I was sure he was enjoying watching me get diapered like an infant. "OK, now for the fun part" said the doctor, "we gotta get this little girl diapered before she has anymore accidents!" Everyone in the room laughed and the doctor continued, "since we know she is a heavy wetter, we are going to double diaper her, and also place a soaker in the first diaper." He pulled out a thick booster pad and had the nurses place it in the diaper. "Don't forget to add some baby powder to the diaper, we don't want baby girl to get a rash on her little tushy!" He directed the last part to me in baby talk and everyone was rolling with laughter. More powder was applied and the nurses grabbed the diaper and brought it up over my privates and belly. It was extremely thick! And they still had to put another diaper on! They taped the diaper and made sure it was on nice and snug. Then the second diaper was unfolded and I almost died of embarrassment when I saw it. It was about three times as thick as the first diaper and it was covered in baby print. The nurses held my legs up so the doctor could slide the massive diaper under me, but before he could my ex chimed in, "doc, do you mind if I put this one on her? I need the practice for a project in my college health class..we are learning how to care for infants and I've never diapered a baby before.." "Well, I don't see why not!" Said the doctor. Oh god, this was mortifying. My ex boyfriend was about to put me in a diaper. I would never live this down! He took the open diaper and slid it under me. Then he took the front and pulled it up over the first diaper. He had to tug on it some to get it to come all the way over the bulk of the first diaper. Then he taped the sides and gave me a couple of pats on the thick diaper bulge. "I can't wait to see you try to waddle around in these!" I started to cry my eyes out. My embarrassment was complete. The doctor and nurses started to file out as my family sat around and discussed who would be helping with my diapering at home for the next few months.
