No nipple just skin. Could draw one on for effect.

My brother would be a good person to ask. When we were kids (I was about 12, he'd have been about 9), we used to skimboard in the back yard, after heavy rain. It was perfect - about 1.5 inches would lay over the entire yard.
But there was an obstacle - a path made from slices of tree trunk, that stuck up just proud of the water. The slices were just a big tree trunk cut horizontally and laid in a path from one side of the yard to the other. Sort of like several rows of - ooooooo
My brother gets the idea to attempt to skim over them. That was his first mistake. His second mistake was actually trying it. He runs as fast as possible and skims toward the path. He fails to lift the front of the board, so it stops dead at the first edge of the path - and he keeps going.
And he basically rips his own tit off. He's laying in the water and mud, flailing around and heaping mud on his tit, in an attempt to stop the pain. Wailing loudly and thrashing around did precisely nothing. There was blood everywhere. It was one of the funniest things I've ever seen.
He ended up with several very deep score marks across his nipple. It grew back but it's sort of fucked up. You can still see the deep scrape marks, which have less pigment.
TIFU reading this comment. Holy shit I have chills all over. Plus that cold feeling at the base of my spine when I’m in a nursing home. Which is ironic considering you can’t nurse with no nipple.
Scar tissue would replace the area. You can see this with some breast cancer survivors.
My nipples feel weird just from reading that question, but to answer the question, just skin.
Those saying "skin" are watching too many superhero movies. Human bodies are not that good at healing themselves. The answer is neither, you'd get a massive scar.
There's a book, by Dr Becker, called the body electric, that might be of interest to you. I believe you can find pdf's online.
A friend of mine, who is now dead, shaved both of his nipples off while shaving his chest whilst drunk in the shower. They both grew back.
