What preposition do you use for the place "the gym"? Every student is in the gym. Every student is at the gym. Which one is right? If both are okay, what's the difference between the two?

"At the gym" is used much more often. "I met Sally at the gym." "How many miles did you run at the gym?" "I like to lift weights at the gym." "In a gym" is used if you're trying to be very specific about something or someone's location. "I met Sally in a gym" could work, but for the other sentences above, it would sound weird.
It probably has something to do with the fact that "home" is a unique place, while "the gym" signify either a specific gym or represents a function (a place where you work out). You can say "I'm at the school" if the school mentioned is not unique.
Synonym for in the gym "I'm exercising in the gym." This specifies that you are inside the building. "I'll meet you at the gym." In this example, you could be meeting someone in the parking lot outside of the gym. You could use these expressions interchangeably though. "In the gym" and "at the gym" are close enough that you would be easily understood.
In this case, "I was at the gym" sounds more natural and is probably more common. But it could depend on the situation. The general rule of thumb is that for larger locations (think city, country, continent) you'd probably use in. "I was at Chicago" sounds odd, but "I was in Chicago" sounds normal. In AmE, it would be at the gym. Certainly you go inside the gym to do Zumba, but it is at the gym (at location) where you do it.

Of the three sentences you provided, the only one that is correct is: What is the name of the gym you attend? You don't train or practise a gym, you train or practise at a gym. Note that this wording is rather formal. You could also ask: What is the name of your gym? Although the gym does not belong to you, this would be perfectly understood. Also: Which gym do you go to?
