Cyanocobalamin and Hydroxocobalamin are commercially available injections in a concentration of 1,000 mcg/mL. Both are available in a 30 mL multiple dose vial. Methylcobalamin injection is not commercially available. It can be compounded by a pharmacy that is capable of preparing sterile injections. Both Cyanocobalamin and Hydroxocobalamin are synthetic forms of B12 and needs to be converted in the body to an active form – Methylcobalamin. My office has an unopened compounded injection vial that is 1 month past its "Do Not Use Beyond" date—can I still use it? You should not use a compounded preparation beyond the labeled "Do Not Use Beyond" date as set forth in the USP Pharmacists' Pharmacopeia guidelines.
What vitamins and minerals can I mix in the same IV infusion bag? The compatibility between ingredients mixed in the same IV infusion bag is dependent upon the chemical nature of each ingredient and on the pH environment of the resulting mixture. Additionally, the osmotic pressure of the IV infusion bag should be either checked or calculated prior to administration. Please call a McGuff pharmacist who can help you decide what you can mix in the same IV infusion bag. My office staff is being told I need to call or fax in patient prescriptions for compounded medications intended to be administered in a doctor’s office. Why is this necessary? A number of states have regulations that do not permit compounded products to be ordered by physicians for office use. All states will permit compounded products to be dispensed for a specific patient once the practitioner orders the product via a patient prescription.
