Good day all,

Safety focus for the day will be on "Lock Out Tag Out (LOTO)". Do spend sometime and read it up, as there will be a recap session tomorrow and random personnel will be asked to share about LOTO and how it is related to their daily routine.


LOTO is implemented to protect against accidental or inadvertent operation when such operation could cause injury to personnel and damage to equipment. There must NEVER BE ANY ATTEMPT to operate any switch,valve, or other energy isolating device when it is locked or tagged out.

In an event that the individual who applied a lock is not there to remove it, his immediate supervisor is permitted to release the lock AFTER CONFIRMING that the work has been COMPLETED and all personnel HAVE CLEARED THE VICINITY of the machine.

LOTO-related record keeping shall include,but not limited to, the following:

a. Training records (attendees names,date, topic,trainer name, sample of training material/syllabus,etc);

b. LOTO device issue records;

c. LOTO activity log(who,what,when,where,why);

d. LOTO device inventory & location;

e. Reports of all LOTO-related incidents; and

f. Master blinder of all Energy Control Procedures (ECPs) for all machines within the unit
