Are patients normally fully nude for dermatology exams?

I have a full body exam every 6 months. The doctor has me strip to my underwear and he checks me everywhere else first. Then he does a quick and awkward “peek-a-boo” around the inside my underwear. After that, he has me stay wearing only my underwear until he’s completely done with anything else he needs to do.
I don’t know if he has everyone wearing only their underwear for so long, but it’s probably pretty obvious to him by my all over tan and my ease at being naked that I don’t mind it. Knowing that skin cancer can and does occur where the sun usually doesn’t shine, I wish he’d check me there as thoroughly as he checks me elsewhere. I’d be perfectly happy to get and stay naked the whole time, but I understand that most people aren’t so at ease about it. And I suspect most doctors and nurses would rather the patients not get and stay naked.

Do female doctors need a nurse in the room when doing a male physical?
Especially if the female doctor is doing a full complete physical and the male patient is naked or if any part of the physical requires the patient to either pull down their pants & underwear and expose their genitalia or bare butt or if the patient has to remove their pants and underwear completely for something specific and maybe the exam is not a full physical and the patient has their shirt on but is having their penis or testicles examined for any reason or maybe the patient is having an anal exam external or internal; or being checked for hemorrhoids, getting a prost

I know this to be true from very recent experience earlier this month (December 2019) my example and short story of the experience is next here.
So, this month I had to go to a clinic I’ve never been to before for a work related reason due to getting a doctors note and clearance to return to work because I had to take off because of a bad weather allergy flare up. I thought this would be a simple 5 minute exam & visit and I would be out. Not the case.
I filled out a super ridiculous long questionnaire and paperwork because I had never been there before. Some of the questions were very specific and detailed and asked for some very personal information. So I answered all the questions including putting some answers that were embarrassing, such as having a few hemorrhoids before several years ago that were removed. It doesn’t bother me one bit however to most, admitting certain personal things like sexual habits or having hemorrhoids and etc… can be embarrassing to most people telling that to a stranger. Ok not to ramble.
So I’m taken into the exam room by a Hispanic female nurse in her 40’s or so, nice lady and we go into a small separate office like room that just had a desk and small table, a scale and equipment for vitals and BP.
She takes my blood pressure and attempted to weigh me, but the scale wasn’t working. We go into the main part of the exam room and she tells me that the doctor will check me over and ask about my allergy and that the doctor will be doing a rectal exam as well, to check to see if any hemorrhoids have returned. So I said oh okay I wasn’t expecting a rectal exam today, but okay.
She then says I will need to remove my pants & underwear. Just to confirm, I asked if I should take them off completely and she said yes off completely but that she wanted to weigh me since the other scale didn’t work and I had already removed my pants so I was in my underwear when she said just keep them on for a minute while she weighs me. So I got weighed in my underwear and went over to the exam table and removed my underwear in front of her and sat on the exam table and she handed me a small white square towel to cover my penis and that’s all. She said okay just wait for the doctor to come in. She leaves.
The doctor comes in and it’s a female. She was from India and was about 50. She talks and asked me about my allergy and listened to my lungs, checked my throat, ears and eyes. She said everything was good and asked me some brief questions. Two or three minutes later she explained her reasons for wanting to do the rectally exam and the importance of being checked regularly to make sure nothing had returned. I said I agreed and understood and was fine getting the exam. What I didn’t know was that she was gonna leave the room and tell me she has to get someone to be in the room with her while she examined my butt and did the rectal exam.
She leaves and comes back with the same nurse from earlier, who already saw me naked but this time would be watching the rectal exam.
the nurse stands in front of the exam table facing me and the doctor gets her gloves and lube and at this point as usual, I’m getting an erection from being naked in front of two ladies and the embarrassment of what is about to happen.
The doctor comes over and asked for the cloth I was covering my penis with and I hand it to her and step down naked and with full erection in front of both of them. The nurse is just standing there staring.
I was expecting to turn around and bend over the table but instead to my surprise she asks me to step up on the foot stool facing them and sit on the edge of the table and lay back on my back, pulling my legs up with my knees to my chest and hold my legs behind my knees. So now I’m super exposed, asshole spread and balls and hard penis in full view as she examined the exterior of my anal area and the inserts her finger inside my butt and does a fairly thorough rectal exam.
She said everything was perfect and looked and felt good, no signs of hemorrhoids at all. She took off the gloves and left as the nurse helped clean me up and wiped the lube off my asshole and said I can get dressed now. I stood up and got down and walked over to get my underwear with my full erect penis flopping around as hard as a rock. She washed her hands and left the room as a pulled up my underwear, making it possible for anyone walking by to see me naked with a hard-on as she opens the door. So long story short, yes in this case because I was naked and received a rectal exam; a nurse had to be present in the room while the doctor did the exam, because she was female as well.

Her exam was not uncomfortable or awkward in any way. It was a “non-event.”

Now let me come to the second part of my answer, which I think more directly addresses the question of male patients feeling uncomfortable with female physicians. My wife is a Urologist. Actually, she was one of the first female urologists in the USA and the very first to graduate from her medical school (a large university in the Midwest US). Many of her professors and colleagues wondered if she would be able to find enough male patients, or if she’d have to restrict her practice to female urology. This is what she found:
Anyway, my wife has a huge general urology practice that’s about evenly distributed between men and women. So I conclude that if a female physician keeps her demeanor professional, her gender will not be a problem for male patients.
